Age With Grace Health Services is in need of a nurse. Read the information below to apply.

DUTIES
• Lead in updating and implementing nursing procedures standard of practice and guidelines according to state ministry of health laws and regulations.
• Oversee and facilitate efficient training process, hands-on skill development and mentorship for Caregiver interns and trainees.
• Develop care plans, coordinate patient-care task and services and mentor,
• Organize recruitment and outreach webinars, workshops, and conferences.
RESPONSIBILITIES
● Assess patients, develop care plan, monitor, and coordinate with medical director to address clients/patents’ health needs.
● Educate patients and family members about their care and health problems.
● Educate patients about their condition, medication, and give them specific instructions.
● Check-in on the patient regularly and evaluate and document their progress.
● Assist the Medical Director with monitoring patients’ treatment plans.
● Provide support for interns participating in the caregiver training.
● Complete basic and advanced caregiver certification course and online resources to keep abreast of updates in geriatrics and home health care.
● Assist with grant writing to secure funding for AWIG services and outreach as required.
● Treat patients with empathy and respect and conduct oneself in a professional manner.
● Comply with organizational policy and state nursing.

Salary is negotiable.

Age With Grace Health Services offers quality Caregiving training and Internship to individuals in Nigeria and in Diaspora. The training comes in two categories:
• An accelerated program: This is done within 6weeks
•Normal program: This is done in 12weeks
