A miscarriage is a term used when a woman loses her pregnancy before reaching the 20th week. This can happen due to various reasons, either related to issues with the developing fetus, like genetic problems or birth defects, or issues with the woman herself, such as problems with her reproductive organs, infections, substance use like co***ne or alcohol, smoking, or injuries.

Symptoms of a miscarriage include bleeding and cramping, which are more likely to occur later in the pregnancy

Statistics show that about 10-15% of recognized pregnancies end in miscarriage. However, it's important to note that many more miscarriages go unrecognized because they occur before a woman even knows she is pregnant. The majority, around 85%, of miscarriages happen during the first 12 weeks of pregnancy, with as many as 25% of all pregnancies ending in miscarriage during this time. The remaining 15% of miscarriages occur between weeks 13 to 20.

Certain factors increase the risk of miscarriage, such as age over 35, a history of previous miscarriages, structural abnormalities in the reproductive organs, smoking, substance use, severe injuries, infections during pregnancy, thyroid problems, poorly controlled diabetes, and certain medical disorders like chronic kidney disease, lupus, and high blood pressure if not appropriately treated during pregnancy. Rh incompatibility, where the pregnant woman has Rh-negative blood and the fetus has Rh-positive blood, also raises the risk of miscarriage.

Cervical cerclage, also referred to as a cervical stitch, is a procedure used to address cervical weakness during pregnancy. This weakness can lead to premature opening and shortening of the cervix, resulting in late miscarriage or preterm birth. Typically performed between weeks 12 to 14, a robust suture is sewn into and around the cervix, offering support. The stitch is later removed towards the end of the pregnancy, minimizing the risk of miscarriage.

Administered under local anesthesia, often via a spinal block, the outpatient procedure is conducted by an obstetrician-gynecologist. Primarily utilized in the first or second trimester, especially for women with a history of late miscarriages, the success rates vary – around 80–90% for elective cerclages and 40–60% for emergency cases. Success is defined by delaying labor and delivery to at least 37 weeks (full term).

Following the procedure, the patient is observed for several hours, sometimes overnight, to check for signs of premature labor. Afterward, she may return home but is advised to limit activity, including abstaining from sexual in*******se, for two to three days to two weeks. Regular follow-up appointments are scheduled to monitor the cervix and stitch, assessing for any indications of premature labor.

For women with a singleton pregnancy at risk of preterm birth, cerclage demonstrates a reduction in preterm births and potentially lowers perinatal mortality when compared to no treatment.
