Episiotomy is a surgical cut on the wall of the opening of the va**na given by the doctor or midwife during labour when the baby's head is visible and the woman pushing to expedite delivery.
It is given when it is obvious the woman will end up with an accidental tear. Local anaesthesia should be given to to prevent pain before the cut and repair of the cut after delivery. It is not true that the labour pain will mask the pain from the cut.

PELVIC ORGAN PROLAPSE
Pelvic organ prolapse refers to descent of one or more pelvic organs (Uterus, Urinary bladder, Re**um, Small bowel) beyond their normal position due to weakness of ligaments and muscles that supports them.
The weakness is usually due to injury sustained during va**nal delivery as the baby passes through the birth canal.
There is descent or prolapse of the pelvic organ initially with the va**na and eventually outside of the va**na.

Types:
The prolapse can be:
1. Utero-va**nal prolapse (Uterus)
2. Cystocele (Urinary bladder)
3. Rectocele (Re**um)
4. Enterocele (Small bowel)
5. Vault prolapse (In people who have previously had surgical removal of their uterus)

Causes:
1. High number of va**nal birth
2. Prolonged labour
3. Big babies
4. Neglected perineal tears
5. Menopause
Etc

Treatment
This depends on type of Pelvic organ prolapse, desire to have more babies, fitness for surgery etc.
1. Pelvic floor exercise can be done
2. Use of va**nal pessaries
3. Use of Oestrogen cream therapy
4. Surgery ( This is the mainstay of treatment)
