Sxx as a method for induction of labour.

It's usually advised that when a woman is term, she should be consistent with sxx, that's if her health permits it. This is because s***m contains prostaglandin that act on the womb thereby stimulating contraction.

Early hours of today, a first time pregnant woman walked into the ward alongside the husband shouting at him, while the husband on the other hand was busy saying " I am sorry".

So it happened that she's term and was told during antenatal classes that sxx helps to stimulate labour.
She got home, told her probably sxx starved husband about it and the man took it personally, while in the process, her water broke.

She was like" nurse the sxx was an aggressive one that's why my water broke".

We sha find a way to calm the situation because the woman was really agitated.

In essence, that we encourage sxx whenever a woman is term doesn't mean men should be aggressive with it. It should be enjoyed by both parties.

Don't make stimulation of contractions the reason for the exercise.

Different types of women in labour;

Those that want your hand in their v@gina every minutes, they probably feel putting your hand there every minutes will help bring the baby out quickly.

They will be like" Nurse please come and check me ooooo", nurse please na, check me, even though it's not been long you just checked them.

The second type are those that don't want you anywhere near their v@gina, even when it's time to check them, they won't allow you. They will be shouting I want to poo poo and still won't open their legs.

I encountered the second type today and ended up taking the delivery with my bare hands....very painful.

Mothers, it's not a crime when you tell your health care providers that you want to seek for second opinion especially when it comes to the result of a diagnostic investigation.

A woman was scheduled for induction of labour, but was asked to go for scan before they can go ahead with induction since she didn't do 36 weeks scan so that the doctors can know how the baby is presenting.

On the day of the induction, she did the scan, brought the result so that doctor can review it.

As the doctor was reviewing the scan, every other thing with the scan was fine but the weight of the baby which was 4.4 kg.

So the doctor told the woman that as a first time mum, she won't allow her go through labour with this type of baby weight as there are lots of complications associated with fetal macrosomia,of which she explained to the patient.

They woman was told she will be worked up for emergency C/S. Immediately, she called her husband and consent was given to go ahead with the procedure.

She was prepared and transferred to the theatre.

Baby was extracted and weighed, lo and behold, She weighed 3.4 kg.

I was angry when I heard it, I even had to confirm it from her page.

My anger was not that she did CS because it's not a death sentence, but this is something a second opinion would have prevented.

In all, a life mother and baby is what a safe delivery is called.

You can always ask your healthcare providers for a second opinion.

" Can I repeat this test somewhere else before we can make a decision" is not a crime.

Happy new month guys, it's been a while.

To our beautiful mothers that their expected date of delivery (EDD) is this month of June, I want you to know that;

Labour is painful, if not very painful.

So, tell yourself that I can do it and I will do it. Be determined to go through the stages.

To our first time mum, you are stronger than you think you are, you can do it.

To our returning mum, people will always say it not your first time, however,no two labour experiences are the same. So tell yourself that " I have done it before and I will do it again".

To those who have done caesarean section before and are for VBAC(vagina birth after caesarean section),that is wanting to try labour this time around, I want you to know that you can do it and you will do it.

Those who their EDD is supposed to be the month of May, but you find yourself still carrying the pregnancy till now.
Do not panic, your health care providers are there for you. This month is your month.

To the mum scheduled for elective caesarean section this month, I want you to know that you are neither less of a woman nor less stronger. Go in there and come out with your bundle (s) of joy.

Congratulations mothers.

Whenever you find yourself in the hospital, try not to hold any information concerning your health back.

Tell the health care provider exactly how you feel, when it started, what triggers it, how long it has been, what you have used.

Just provide them with the needed information, tell them everything, you don't have to be ashamed.

This will help them care for you better.

There's no need hiding any information from them if you seek to be cared for.

SITZ BATH

A woman was rushed into the hospital early hours of today bleeding profusely, like the type of bleeding seen in cervical tear. We actually thought it was cervical tear.

It was later deduced that she delivered a week ago and had episiotomy during delivery, which was sutured.

On discharge, she was taught on how to care for the wound using lukewarm water and salt ( sitz bath), however, when she got home, based on advice from many people, she started using hot water for the sitz bath which led the suture use for the wound to loose ( wound break down)

Two days before she was rushed to the hospital, she started bleeding but still didn't come to the hospital and was still doing sitz bath with hot water. She even used the hot water a day before she was brought to the hospital. Honest patient..... lol.

It was when the bleeding became profuse that she was then rushed to the hospital. By that time, she had already lost so much blood.

The episiotomy was repaired the second time and she had to take two pints of blood as the blood level was very low (20%), she was also placed on antibiotics as there were lot of de@d tissue in the v@gina due to loss of blood supply to that area.
The de@d tissue were also cut off ( debridement).

You can imagine what she went through.

How is sitz bath done?

1) Get a sitz bath bowl and if you cannot afford it, you can get a normal bowl ( the one as big as the sitz bath bowl)
2) Add cold water into the bowl then start diluting it with hot water. The temperature of the water should be that type you can bathe with without having to shout while bathing ( lukewarm)
3) Add enough salt into the water. The salt should be added in a way that you will start seeing undissolved salt in the water. Dettol or savlon is optional as the main thing needed for it is enough salt.
4) Sit inside the water for about 15-30 minutes. You should sit in a way that all your buttocks is immersed in the water.
5) This process should be done three times in a day for 10 to 14 days.
6) Also, sitz bath should be done each time you use the toilet to prevent faecal matter ( poo) from sticking to the wound site as this can also cause the wound to break down.
7) Always keep the wound area clean and dry.

Truth is, repairing of episiotomy wound the second time can be very painful more than the first time the wound was sutured.

Let's be guided so as not to fall victim of this.

BIRTH PREPAREDNESS

While pregnant, one of the things to prepare for is transportation on how to get to hospital incase there is need for you to visit the hospital regardless of the time. Also how to get to the hospital when labour starts , whether in the midnight, whether it is raining or your husband happens not to be around when it starts.

By doing this, it helps you access the hospital on time and also get timely care because there are some cases during pregnancy that requires urgent and immediate attention.

A woman who was already due for delivery was at home one evening when her water broke, however, she didn't start coming to the hospital but was waiting for the husband to come back from work with the car so that she can be taken to the hospital.

When the husband eventually came back from work, she was brought to the hospital. When she arrived the hospital, some questions were asked and she told them she has seen water 5 hours ago. They did her vitals and hers was fine, when it was time to check for baby's heartbeat, they couldn't find it.

V@gina examination was done and it was at that point that they discovered that the baby cord has slipped out( cord prolapse), the reason they couldn't find the baby's heart beat, the baby was already d@ad.

One of the signs you don't joke with in pregnancy is liquor drainage ( breaking of your baby's water). As soon as you see water, start running to the hospital as far as your legs can carry you.

Had it been she came earlier, I am sure that case of cord prolapse would have been prevented.

Had it been while preparing for their birth plan, they included alternative means of transportation to the hospital incase the husband happens not to be around when labour starts, she would have gone home with her healthy baby.

So please, while planning on how to get that beautiful box you will use to pack your baby's things while going to the hospital,means of transportation should not be left out.

STILL ON BEING HEALTH CONSCIOUS...

Before I gained admission into tertiary institution, I was always having this lower abdominal pain.

The pain became so intense that I started getting worried because I knew it can't be an ordinary pain.

Fortunately, among the fees I paid to the school, health insurance fee was among , which means I can access care in the school hospital and I learnt the fee expires after one year.

That fateful day, I went to see the doctor and series of assessment were done. The funniest thing was that the doctor ask me to point to the place I feel the pain, when I did, he palpated asking me if I feel the pain while he was palpating, but I said no as I honestly do not feel any pain while he was.

I was sent for abdomino-pelvic ultrasound scan that day, and when the result came out, I was diagnosed of;

Right ovarian cyst
Endometritis and
? Pelvic inflammatory disease (PID).

You need to see the way I shouted. I was like " only me all these diseases"?

When I went back to the doctor, he said the cyst would be removed via surgery, however, he's going to place me on some medications.

When he wrote those medications, he asked me to make sure I get the original of each medication and I am to take the medication for 14 days.

I was also ask to go for a repeat scan after completing the medications.

As a student, I got busy with school activities and didn't remember to go for the repeat scan.

Eventually, I went for the repeat scan and was certify free of;

Right ovarian cyst
Endometritis and
? PID.

From that time till now, I have done more than five pelvic scan and all usually come out fine ( no anomaly detected)

What point am I trying to prove?

Had it been I ignored those pains till now and didn't seek medical assistance, what would have become of me?

I would have developed complications that comes with the aforementioned conditions.

So, please take that bold step today and find out why you have been having those signs and symptoms.

LET'S BE HEALTH CONSCIOUS

No sign or symptoms in the body should be ignored.

As a lady, you noticed your flow is usually heavy, irregular, extremely painful, there are even cases where you don't see your period for some months and you happen not to be pregnant, to mention but few and you didn't deem it fit to consult your health care personnel.

It is wise to seek for medical attention whenever something feels off about you as a lady medically.

Guessing or thinking it's normal won't do you any good. Go to the hospital, get checked and let the doctors certify you ok.
Some of this symptoms are usually an indication that there is an underlying gynaecological condition, hence early detection helps In prompt treatment and prevents the condition from aggravating.

There was this lady, according to her, her me**es does not come every month; some time, she sees her period three times in a year and her friends are always seeing theirs every month and she thought to herself that this is not normal which prompted her to seek for medical help.

When she got to the hospital, series of investigations were done and she was diagnosed of polycystic ovarian syndrome ( PCOS) (A topic for another day.

PCOS happens to be one of the causes of infertility. Having been diagnosed of this, she was commenced on treatment even though she was not yet married then. She continued her treatment modalities till she gets married.
After getting married, she got pregnant the next month.

What point am I trying to prove; had it been she ignored the fact that she does not menstruate every month, her wake up call would have been when she has gotten married , who knows it might have taken her many years to conceive.

In essence, do not ignore any symptoms, get checked to know what is happening, there's no harm in getting checked.

Please, let's take care of our health and let's be health conscious.

LAST MENSTRUAL PERIOD ( LMP)

LMP is the date of the first day your last period came not the date of the last day of your period in the previous month. For instance, if February me**es started on the 10th and ended on the 14th, the LMP in this case is 10th of February and not 14th.

Have seen cases of patient/client being ask about their LMP and they will be like" I think it came out around on the 2nd, 3rd or 4th, I am not really sure of the date it started but it's within this three days.

Recalling LMP off hand can be difficult for some people.There are even cases where you will need to remember the one before the last period, i.e, the health personnel might need you to remember not only that of February but also that of January.

So how do you go about it in this case?

In other to avail yourself of all this stress of having to recall off hand, while not get yourself a diary, a jotter or a small calendar where you chart the day your me**es started and the day it ended

This will even help you take note if there is variation in the number of days the periods last, also helps to calculate the ovulation period.

Today, I am encouraging you to get yourself a jotter, diary or a small calendar if you haven't gotten any yet.

Below is what I use in keeping track of my menstrual period.
