The global incidence and prevalence of impotence among men between 30 and 80 years old is high and worrisome. As we age, we lose muscle and lean body mass, the vessels become flaccid or inflexible losing both their elasticity and tensile strength and so is the decrease in amount and force of the ej*****te and the need to ej*****te also decrease with age but the capacity for er****on is retained and a man’s s*xual virility can be retained unaffected well into his 80s.
THE MALE S*XUAL FUNCTION
The male s*xual act can be initiated by psychological and/or physical stimulation. Thus simply thinking or having s*xual thoughts or dreaming s*xual in*******se [like the common experience that occurs especially during teen years, which cause nocturnal emissions (wet dreams)] can lead to an er****on and even ej*******on. This is simply a hormonal play and a physiological phenomenon that requires no spiritual consultation. Therefore, s*xual fantasies or erotic imaginations and other psychological factors arouse s*xual response in men but they may not be sufficient for the performance of the s*xual act whereas ge***al stimulation can lead to er****on and ej*******on through inherent reflex mechanism without any psychological stimulus. That is why some persons with spinal cord injury can still perform. Spinal cord injury prevents the transmission of nerve impulses from the brain.
MECHANISM OF ACTION
Stimulating the sensitive tissues of the p***s or p***c region sends nerve impulses to the spinal cord, causing a reflex impulse to the p***s, which leads to dilation of the arteries and bulging, puffiness and enlargement of the erectile tissue, secretion of mucus by the urethral glands to lubricate the urethra, which makes in*******se more pleasurable. The initial phase of this action is regulated by the parasympathetic nervous system (PNS), which relaxes the body after periods of stress and controls secretions including those of digestion, and regulates bodily functions such as breathing, visualization, meditation, sleep and heart rate during rest or relaxation. While the PNS is responsible for er****on and lubrication, the sympathetic nervous system (SNS) champions the emission and ej*******on, which are the zenith of the male s*xual act. At the peak and high intensity of the s*xual stimulation, the reflex centers of the spinal cord begin to emit sympathetic nerve impulses that initiate emission and eventually ej*******on occurs.
Emission begins with the contraction of the vas deferens, which is the tubule that transports the s***m from the epididymis to the prostate. This contraction leads to the expulsion of s***m into the ejaculatory duct and then the urethra. Contractions of the prostate and seminal vesicles also expel prostatic and seminal fluid into the ejaculatory duct thus propelling the s***m into the urethra. All the fluids are mixed in the internal urethra along with the secretions of the urethral glands to form semen.
Ej*******on starts with the filling of the urethra, which elicits sensory nerve impulses that further excite the rhythmic contractions of the internal organs and also causing the rhythmic contraction of the erectile tissues. Together, these contractions cause a tremendous increase in pressure, which releases the semen from the urethra. The pelvic and abdominal muscles are also involved causing thrusting movements of the pelvis and p***s, which help propel the semen. The entire process of emission and ej*******on is known as the male or**sm. But after ej*******on, the male s*xual excitement disappears almost entirely within one or two minutes and er****on disappears.
DIAGNOSIS OF ED
The conventional practice of diagnosing the causes of ED requires a medical history and information on any prescribed drugs, physical examination, scanning imagery and ultrasounds, laboratory tests and investigations like complete blood count and urinalysis, biochemical profile, glucose tolerance test, and serum hormone levels, psychological evaluation, nighttime pe**le monitoring and neurological examination.
Taking biochemical profiling, for instance, when the total blood cholesterol level is checked and found to be above 200mg/dl it is considered a strong indicator for atherosclerosis causing decreased blood flow, which physicians suggests could be established during evaluation by injecting prostaglandin E1 (PGE1) or papaverine into the p***s because they cause the arteries to dilate and deliver more blood to the erectile tissues resulting in er****on. If the er****on occurs but could not be maintained, then it is suggested that a venous leakage may be the case and surgery is recommended.
CAUSES OF ED
Causes of ED are multi-factorial but mainly grouped into organic and psychogenic factors with majority being organic due to physiological issues, which accounts for over 90% of cases in men above the age of 50 years. Organic (about 85% of cases and largely due to vascular insufficiency caused by atherosclerosis, pelvic surgery, pelvic trauma, and venous shunting), drugs (antihistamines, antihypertensives, anticholinergics, antidepressants, antipsychotics, tranquilizers, and others), alcohol and to***co, endocrine disorders (diabetes, hypothyroidism, decreased male s*x hormones, elevated prolactin levels, high serum estrogen levels), diseases or trauma to male s*xual organs (disease of the p***s, prostate disorders), neurological diseases (pelvic trauma, pelvic surgery, multiple sclerosis), and psychological (10% of cases) as well as psychiatric illness, stress, performance anxiety and depression while 5% of the cases are due to unknown etiologies.
The most common diseases of the male s*xual organs affect the p***s (e.g. Peyronie’s disease) and the prostate (e.g. benign prostatic hyperplasia, BPH). Peyronie’s disease is characterized by the thickening of the pe**le sheath (the fibrous connective tissue) that causes the p***s to bend at an angle during an er****on, which makes in*******se difficult and painful while BPH is simply an enlargement of the prostate gland that affects its functions.

In addition to zinc, magnesium, manganese, selenium, essential fatty acids, beta carotene, vitamins A, B complex (especially B6), C and E are key to male s*xual function. It is important to take the whole vitamin C complex or supplements containing chelated ascorbate 500 – 1,000 mg with citrus bioflavonoids, acerola and rosehips three times a day not just ascorbic acid and a natural vitamin E 400 – 800 iu a day rather than taking the synthetic dl-alpha tocopherol acetate. Dietary supplements for ED include dehydroepiandrosterone (DHEA), which increases libido in women and helps ED in men, L-arginine that stimulates dilation of the blood vessels via nitric oxide, and L-carnitine, which helps the body turn fat into energy.
Exercise and herbal teas are part of the natural remedy for ED. Exercise is important because it improves circulation providing the body with sufficient oxygen and improves resistance, endurance, s*xual performance and better physical fitness.

It can cause hyperactivity due to hyperstimulation, nervous breakdown, vision damage, general body poisoning, exhaustion, insomnia, high blood pressure, irregular heartbeat, heart attack, agitation, dizziness, headaches, skin flushing, anxiety, psychosis in persons with schizophrenia, panic attacks, hallucinations, worsening BPH, urine retention, liver damage and increasing risk of bleeding. The U.S. Food and Drug Administration (FDA) approved the alkaloid, yohimbine, which is the active pharmaceutical ingredient (API) contained in johimbe for the treatment of impotence with the claims that yohimbine hydrochloride increases libido by increasing blood flow to the erectile tissue without any affect on the hormone (testosterone) levels but it classified yohimbe, which is the source of yohimbine as an unsafe herb. Whether you intend to use yohimbine hydrochloride or johimbe bark, we strongly advise you to apply caution or better still avoid using it.
Potency wood or muira puama (Ptychopetalum olacoides) is also aphrodisiac and a nerve stimulant common in Brazil.
Ginkgo biloba extract is used to increase blood flow and oxygen to the brain. This can be taken 60 to 80 mg a day for 6 – 8 weeks and up to 12 – 18 months. It enhances blood flow through both arteries and veins without any change in systemic blood pressure.
Damiana (Turnera diffusa) works by irritating the urethra and causing sensitivity of the p***s. Just a cup of damiana tea a day.
Panax ginseng or Korean red ginseng promotes growth of the te**es, increase s***m production and testosterone levels. It may also improve stamina, concentration, stress, and general well-being. It is an antioxidant, release nitric oxide (NO) but should not be used for more than 6 – 8 weeks, use in a cyclic or alternate pattern (2 - 4 weeks and a break for 2 weeks). Some persons may be allergic to it and it can cause insomnia.
Chasteberry (Vitex agnus costus) primarily used to treat premenstrual syndrome and high prolactin in women. It can also lower prolactin level in men at a dose of 480 mg per day.
Maca (Lepidium meyenii) may alleviate stress but it can also elevate blood pressure. It also contains amino acids, iodine, iron and magnesium.
White’s ginger (Mondia whitei) increases libido and improves low s***m count, increases s*xual desire, s***m motility, testosterone levels, nitric oxide production and er****on. The common ginger is also applied for same reasons. And when combined with garlic and onions (white or yellow type), they improve cholesterol by reducing low density lipoprotein-cholesterol (LDL-C) and increasing high density lipoprotein-cholesterol (HDL-C) and thinning the blood thereby making it flow better among other qualities they possess.
Other herbs used include h***y goat w**d (epimedium) a natural phosphodiesterase inhibitor that dilate blood vessels. Epimedium may affect heart or breathing function. Musli (Chlorophytum borivillanum) has anti-inflammatory properties and help increase s*xual desire. Saffron (Crocus sativus) can improve s*xual dysfunction and overall s*xual satisfaction. Tribulus terrestris increase amount of nitric oxide in the cavernous tissue of the p***s, and Pygeum africanum helps reduce enlarged prostate. Peppers and citrus peels thin the blood and supply vitamin C.
Reversing atherosclerosis is a primary focus for treating ED since it is responsible for most of the cases of ED. An optimum, adequate diet is recommended to prevent or reverse atherosclerosis and blockage of clogged arteries. Lysine and chelated niacin (vitamin B3) e.g. inositol hexaniacinate (1,000 – 3,000 mg a day) has been found to be helpful in regulating cholesterol levels and contributing to blood sugar control, and improving blood flow (circulation).
Impotence can be a major cause of mental and emotional stress in men because it brings on frustration, anxiety, and low self esteem. Therefore, stress reduction techniques, such as, relaxation exercises, reflexology, and deep-breathing can help resolve anxiety. Psychotherapy for depression and other psychological factors may be beneficial to men with ED. But we do not advise or recommend the use of pe**le prosthetics and vacuum constrictive devices, which are common place in conventional orthodox practices.
CONCLUSION
