MANAGEMENT OF FIBROID

This involves Non- surgical approach and the surgical approach.
1. THE SURGICAL APPROACH : this includes the following;
* WAIT AND WATCH: for asymptomatic Fibroids, a doctor may recommend monitoring their growth.

* MEDICATIONS: such as pain relief, NSAID eg ibuprofen, narrowed and hormonal drugs may be given to you.
GnRH agonist, : these medications temporarily shrink Fibroids by including a menopausal state. But typically, used for short term treatment before surgery.

* MRI GUIDED ULTRASOUND SURGERY(MRGFUS):
Uses focused ultrasound waves to destroy Fibroid tissue with invasiveness.

* RADIOFREQUENCY ABLATION (MYOLYSIS): here. heat is used to destroy Fibroid, often through a minimal invasive procedure.

3. SURGICAL OPTIONS:

RISK FACTORS TO UTERINE FIBROIDS
BIRTH CONTROL: Using birth control pill before tye age of 16 may be linked with a higher risk.

RACE: All people of reproductive age who were born female. Although black race are more likely to have Fibroidthan other racial group.

COMPLICATIONS OF FIBROID IN PREGNANCY
Fibroid don't interfere with getting pregnant. Although, some Fibroids such as the submucosal layer Fibroid could cause infertility or pregnancy loss.
Fibroid also may raise the risk of certain pregnancy such as:

* ABRUPTIO PLACENTA: A situation where the placenta separates from the inner wall of the uterus before the expected date of delivery.

* FETAL GROWTH RESTRICTION:When an unborn baby in the uterus doesn't grow as well as expected before date of delivery.

* PRETERM DELIVERY/ PREMATURE BIRTH: The baby is born too early before 37th week of pregnancy.

PREVENTION OF FIBROID/ MYOMA
Fibroid growth is hereditary however ,you may be able to lower your Fibroid risk with healthy life style changes such as:

CAUSES OF UTERINE FIBROIDS/ MYOMA
it is unclear, why Fibroids develops ,however, some factors may influence it.such as:

1. HORMONAL FACTORS:
The ovaries produce estrogen and progesterone. These hormones cause the uterine lining to regenerate during each menstrual cycles and stimulates the growth of Fibroids.

* PREGNANCY: Pregnancy increases the chances of Fibroids growth. During pregnancy ,estrogen and progesterone are produced in a large quantity which makes Fibroids develop and grow rapidly.

* EARLY MENSTRUATION: this increases the chances of Fibroids and menopause.

* MENOPAUSE: Fibroid tends to shrink significantly after menopause because hormone level drops.

2. GENETIC AND BIOLOGICAL FACTORS

*HEREDITARY: family history shows evidence that it runs in families.

*. GENETIC CHANGES: Many Fibroids contain genetic changes that are different from normal uterine muscle cells.

*. STEM CELL ORIGIN: Many Fibroids may start from stem cell origin of smooth muscle tissue of the uterus and divides repeatedly.

LIFE STYLE AND OTHER FACTORS

*.AGE: Fibroids tend to grow in certain child bearing age women,basically from 30years and above

UTERINE FIBROID/ MYOMA

Fibroids are common growth of the uterus. They often appear during child bearing year which is from 25-49 years.

Fibroids are abnormal growth that develops in the uterus of a woman.
Sometimes these tumors become quite large and cause severe abdominal pains and heavy menstrual flow/ bleeding. In most cases, they are asymptomatic and shows no sign at all.

These growths are typically benign or noncancerous.

TYPES OF FIBROIDS/MYOMA

1. INTRAMURAL FIBROIDS: these growth are more common in women.and appear on the muscular wall of the uterus. Intramural Fibroids may grow larger and stretch the uterus there by preventing conception.
2. SUBSEROUS FIBROIDS: They form on the outside of the uterus which is called serosa .they may grow larger to make the uterus appear bigger on one side.
3. PEDUNCULATED FIBROIDS: here, subserosal Fibroids may grow on the stem,a slender base that supports the tumor.when this happens, It is called pedunculated Fibroids.
4. SUBMUCOSAL FIBROIDS: These Fibroids are not common,they grow on the myometruim ,the middle muscle layer of the uterus.

5. CERVICAL FIBROIDS: These Fibroids develops on the cervix, and can prevent conception.

SYMPTOMS OF UTERINE FIBROIDS/ MYOMA

Estrogen and progesterone hormones, stimulates the growth of Fibroids in a woman.
The symptoms one may have depends on the position of the Fibroid and menstruation that would happy
1. Heavy bleeding between or during periods.
2 . Menstrual clot
3. Menstruation that last more than normal.

HEPATITIS B VIRUS INFECTION ( A global health problem)

Hepatitis B is an infection of the liver caused by hepatitis B virus.the infection can be acute (short term) or chronic ( longterm).
Hepatitis B can cause chronic infection of the liver such as liver cirrhosis and liver cancer which would put one at risk of death.

TRANSMISSION OF HEPATITIS B VIRUS
Hepatitis be can spread through the following means:
* Body fluids - such as : Blood, saliva,vaginal fluids and semen.
* Childbirth- Mother to child transmission during delivery. Hence the importance of giving new born babies hepatitis B vaccine soon after birth.
* Through needle stick injury: hazzard both in the hospital settings and at home.
* Tatooing
* Piercings : both ear,nose piercing etc
* Transmission through sharing or reuse of contaminated needles 🪡 and syringes 💉 .

Hepatitis B acquired in adulthood leads to chronic hepatitis in less that 5%cases , whereas ,hepatitis B infection in infancy and early childhood leads to chronic hepatitis in about 98%.
This is the basis for prioritizing infant and childhood vaccination.

SYMPTOMS OF HEPATITIS B VIRUS INFECTION

Most people do not experience any symptoms when newly infected. It is asymptomatic. Some people have acute illness with symptoms that last several weeks. Such as :

* yellowing of the skin and eyes(Jaundice)
* dark urine
* feeling very tired
* nausea and vomiting 🤢
* pain in the abdomen.

When severe ,acute hepatitis can lead to liver failure which can also lead to death.

DIAGNOSIS OF HEPATITIS B INFECTION
This can only be diagnosed in a medical laboratory with a blood sample.
Hepatitis B virus is equally a coexisting disease with HIV.

PREVENTION OF HAPATITIS B VIRUS INFECTION
* Hepatitis B virus infection can be prevented with a safe and effective vaccine. The vaccine is usually given soon after birth with boosters and few weeks later.it offers a 💯 protection against the virus.

TREATMENT OF HEPATITIS B VIRUS INFECTION

There are no specific treatments for hepatitis B.

* people with acute hepatitis B VIRUS should focus on eating a healthy diet and drink plenty of fluids to prevent dehydration from vomiting and stollinh.
* Chronic hepatitis B can be treated with oral antiviral medications 💊 such as Tenofovir and Enetevir .
These medications can :
a. Slow the advance of liver cirrhosis
b. Reduce cases of liver cancer
C: Improve long term of survival.

Get tested for Hepatitis B virus, if you are confirmed negative please, get vaccinated immediately, but if you are confirmed positive, please commence treatment immediately. Don't be ignorant of this Hepatitis B virus infection.
Prevention is better than cure.
