Understanding Azoos***mia and FSH Levels

In men with non-obstructive azoos***mia (NOA), high FSH (follicle-stimulating hormone) levels are often seen—but they are not the cause of the problem.

High FSH is the body’s natural response to testicular failure, trying to stimulate s***m production when the testicles are not functioning properly. This condition is known as primary testicular failure.

It’s important to understand: reducing FSH will not restore s***m production, because FSH is a consequence, not the cause, of azoos***mia.

What does high FSH mean in azoos***mia?
It means non-obstructive azoos***mia—specifically, primary testicular failure.
When FSH is high, the testicles aren’t producing s***m properly.
If FSH is normal, more tests are needed to know the cause.

Can Prolistem Help in AZF Genetic Disorders?

Azoos***mia caused by AZF deletions (AZFa, AZFb, and AZFc) is one of the most difficult forms of male infertility.

While AZFa is typically linked to a complete absence of s***m, men with AZFb or AZFc deletions may still have isolated areas of s***m production.

Prolistem is designed to activate these remaining active tubules. In some cases of AZFb and AZFc, it may improve the chances of finding s***m—especially when followed by microTESE.

Each case is unique, but even with genetic causes, there is still room for hope.

Question:
How many microTESE procedures can a man with azoos***mia safely undergo during his fertility journey?

Answer:
There is no strict limit on the number of microTESE procedures a man can undergo, but most experts recommend no more than 2–3 attempts in a lifetime.

Recent studies and clinical guidelines suggest that each microTESE can cause scarring and reduce testicular function, especially in men with non-obstructive azoos***mia (NOA). The success rate is highest during the first or second procedure, particularly when combined with treatments like Prolistem® that improve the chances of finding s***m.

Before repeating microTESE, it’s important to:
• Evaluate hormone levels (FSH, LH, testosterone)
• Allow enough time for testicular recovery (at least 6–12 months between surgeries)
• Consider treatments that may stimulate remaining s***m production.

Always consult a fertility specialist to plan the best approach based on your unique case.

FAQ: Does testosterone reduction impact my health?
With Prolistem®, testosterone is reduced by only 15–20%—just enough to stimulate the body’s backup s***m production pathway.
This small, natural reduction does not affect overall health, energy, or libido.
It’s a safe and targeted strategy to help men with azoos***mia produce s***m for IVF.

What is Prolistem?
Prolistem is a natural, science-backed treatment designed to reactivate s***matogonial
stem cells and restore s***m production in men with non-obstructive azoos***mia.
During the six-month treatment, the first three months focus on the S***matogenesis
Restarting Process—temporarily lowering testosterone levels to create the ideal
environment for s***m development.
In the final three months, testosterone levels return to normal, supporting healthy s***m
production.

Unlocking the Genetic Code of Male Fertility

S***m production is a complex process influenced by many genes, and we are only beginning to uncover their secrets. For instance, a recently identified gene, PNLDC1, plays a crucial role in stabilizing small RNAs (piRNAs) that protect s***m cells from genetic damage. Mutations in this gene can disrupt s***m production, leading to infertility.

This discovery is a reminder that countless other genes impacting male fertility are still waiting to be found. Advancing research in this field holds the promise of better diagnostics and treatments for male infertility.

Prolistem's primary objective in phase one is to reduce testosterone levels, activate Sertoli cells, and enhance non-classical pathways. The outcome? A distinct signal to encourage the growth and maturation of s***m-producing cells. ***mia
