17 with localized asymptomatic AP and already initiated endodontic treatment.

Tooth was tilted to the B, P canal had also a bucally curved apical part.

Managed to make some space on the inner curvature of the P canal (buccally thus) and with BTR pen removed the file.

The MB and DB canals were much more difficult -> long and curved apically.

NaOCl and EDTA irrigation.

Shaped up to VS FlexiGold 3% 025 in all 3 canals.

Single cone GP with Bioceramic sealer and then composite resin filling.

Apparently a boring upper 7 with D decay under the amalgam filling and partial necrosis.

CBCT revelead abrupt curve of the DB canal, merging MB and P roots, deep D decay.

Caries removal, gingivectomy, D build up, access and shaping of the the 3 canals using handfiles 006-008-010 and then VA Flexi Gold up to 020/.03 in all canals.

NaOCl and EDTA irrigation, Single cone GP and BC sealer.

The winner takes (most) of the apical curves ;-)

Tooth 27 — treatment under a 41-year-old bridge. One of those cases where CBCT + microscope = the only way forward. 27 with localized painful AP

Main challenges:
• Working through an old porcelain–metal bridge, with limited opening.
• Severe palatal decay reaching the bridge margin — cleaning it without dislodging the bridge was a constant risk.
• A completely calcified pulp chamber hiding the P canal.
• C shaped buccal canal with a purely apical split into two canals (visible only on CBCT).
• Very narrow palatal access and restricted visibility due to the prosthetic design.
- huge decay P - needed to clean until the exterior part of the bridge (repaired with GIC and composite)

Despite this, all canals were negotiated, shaped and cleaned, and the apical bifurcation was fully managed.
Shaped with VS Flexi Gold 3% up to 020/.03%.

Next steps:
Because of the deep decay under the bridge, I recommended:
• Removal of the old bridge
• Implant for tooth 26
• New build-up + single crown for 27 for long-term stability

NaOCl irrigation/ Single guttapercha filling with BC sealer.

Localized symptomatic AP tooth 27.

MB root curved apically last 3 mm.

Crown placed few months ago.

Very limited mouth opening -15 mm

Rotary shaping of all 4 canals - up to VS FlexiGold 20/.03

Difficulties finding a proper conefit P root. 3 adjustments necessary. Single cone GP and BC sealer.

Access filled with composite.
