Calcium Pyrophosphate Dihydrate (CPPD) crystal-related arthropathies cause acute and chronic arthritis due to calcium pyrophosphate crystal deposition in joints and soft tissues, leading to inflammation and damage. It is the third most common inflammatory arthritis, more prevalent with age. Varied clinical presentations hinder diagnosis, contributing to diagnostic uncertainty. The European Alliance of Associations for Rheumatology (EULAR) identifies four clinical presentations: 1) asymptomatic CPPD; 2) osteoarthritis (OA) with CPPD; 3) acute CPP crystal arthritis; 4) chronic CPP inflammatory crystal arthritis.

Diagnosing and treating CPPD arthropathy pose numerous challenges. Correct diagnosis is the initial hurdle, with synovial fluid aspiration and examination under PLM facilitating diagnosis. Including white cell count, gram stain, and fluid culture helps exclude other diagnoses, particularly infection. Despite imperfections, x-ray and US are the most useful imaging techniques, identifying chondrocalcinosis deposition in hyaline or articular cartilage. Identifying and treating associated conditions is crucial.
