HØKH - Health Services Research Unit - Akershus University Hospital These include RCTs, intervention studies, observational studies, epidemiological studies, studies based on national registries, and qualitative studies .

The Health Services Research Unit (HØKH) is a vibrant unit with 30 researchers, around half are PhD candidates, and comprises expertise in medicine, nursing, health economics, psychology, sociology, anthropology, statistics, and mathematical modelling. Our research is clustered around three overarching themes, Organisation, Integration and Resource Use, Operational Research, and Systems Improvemen

t, we currently hold 15 large research grants and participate in a further 15 together with national and international consortia and collaborators. The beginning of HØKH:

HØKH was established as a regional research and competence centre in health services research at Akershus University Hospital in 2005. Situated within one of the largest general hospitals in Norway, HØKH has established itself nationally as a leading health services research centre and our research is designed to be patient centred and of clinical relevance.. Initially, most of our studies focused on specialist health services, but today they span across different service levels and on the collaboration between these levels. We employ multi-disciplinary approaches, and we build on, or develop, both qualitative and quantitative methods to address our specific research questions. Over the past ten years, more than 50% of our funding has come from competitive research grants. We publish between 50 and 60 papers in international referee based journals annually, and 2-4 of our PhD students successfully complete their doctorates each year. Since 2013 HØKH has been the coordinating unit for the Norwegian Health Services Research Network .

Gørill Birkeli og veilederne, bl.a. HØKHs Ellen Tveter Deilkås har publiserte en systematisk litteraturstudie om verktøy som kan benyttes til å lære av positive erfaringer av hvordan helsetjenesten fungerer i praksis. De fleste studiene handlet om validering av verktøyenes evne til å gi innsikt i praktiske erfaringer, og til å forbedre medarbeideres opplevelse av velvære. Ingrid Oma Langeland og veilederne, bl.a. HØKHs Ellen Delikås har publiserte en studie om hvordan spørreskjemaet Safety Attitudes Questionnaire- Ambulatory Version (SAQ-A) fungerer på helsestasjoner og i skolehelsetjeneste. Gørill Birkeli og veilederne, bl.a. HØKHs Ellen Tveter Delikås, har gjort en kvalitativ studie av hvordan helsepersonell erfarer å bruk verktøyet «Råbra» for å lære av positive jobberfaringer. Studien viser at verktøyet kan bidra til å øke motivasjon og spre gode erfaringer, men kan også gi opphav til triste opplevelser av å ikke bli sett. Erfaringene som ble delt ble unntaksvis brukt til forbedringer, da de sjelden ble brukt til analyse og organisatoriske læring. Les mer her:

HØKHs Rugkåsa og kollegaer testet, i en klynge-RCT, ReCoN-intervensjonen rettet mot kommunal) psykiske helsetjenester for å redusere tvangsinnleggelser. Hovedhypotesen om færre tvangsinnleggelser i intervensjonsgruppen sammenlignet med kontrollgruppen ble støttet. Sekundære hypoteser om mer langsiktig effekt og forskjeller i henvisninger ble ikke støttet. Resultatene utgjør «proof of concept» for at kommunalt psykisk helsearbeid kan bidra til politiske mål om redusert bruk av tvang. Videre testing i mer heterogene kontekster er nødvendig. Herman Bjørnstad and colleagues has explored how older heart failure patients disclose contextual adherence-related factors during clinical interactions. Analyzing 105 audio-recorded visits, researchers identified 742 instances of patients referring to established adherence factors. Medication-related concerns were the most impactful barriers to adherence for this patient. Read the study here:

They found that although doctors often addressed patients' problem disclosures, half remained unresolved at the end of the consultation, usually because patients indicated that the doctor’s adherence support suggestions were against their preference.
