Untreated renal stone disease is one of the most prevalent cause of preventable kidney failure in our part of the world.

73-year female came to us with Urosepsis due to right pyonephrosis, peri-nephric abscess and pyomyositis which was initially managed with PCN. 6 weeks later, she was admitted for Right open nephrectomy which was very challenging. She had post-operative wound infection as anticipated but was managed with regular dressing and finally delayed primary closure.

Bedwetting or nocturnal enuresis भनेको क हो?

५ वर्ष सम्मका धेरै बच्चाहरुले रति ओछ्यानमा पिसाब फेर्ने गर्छन। ७ वर्षको उमेर सम्म १५% बच्चाहरुले फेर्ने गर्छन।यसलाई bed wetting भनिन्छ। यो बच्चाले पिसाब थैली को नियेंत्रण गर्न नसक्नु को कारण हुन्छ।

७ वर्ष भन्दा ठुलो उमेर सम्म पनि ओछ्यानमै पिसाब फेर्ने बच्चालाई सो को कारण पत्ता लगाई उपचार गराउदा ठिक हुन्छ।

Narrowing of the the urethra (Urethral stricture disease) is seen in upto 20% of patients (both males and females) presenting with weak urinary stream or LUTS. The leading cause of stricture is infection (Sexually Transmitted Disease) followed by inflammation and trauma.

Non-contrast CT (NCCT) has the best chance of detecting a urinary tract stone with sensitivity nearing 100% for stones more than 3mm (USG has sensitivity around 45%). Nevertheless, USG is the first line of investigation because of its safety and ease.

Can you find the stone in this NCCT sequence?
Hint: Look for left kidney, try to follow ureter nearing the bladder.

साधारणतया,
४ मिलिमिटर भन्दा सानो - ७५% देखि ९०% सम्म
५ देखि ९ मिलिमिटर - ६०% सम्म
९ मिलिमिटर भन्दा ठुलो पत्थरी आफै निस्किने सम्भावना धेरै कम हुन्छ

तर यो अवस्थाहरुमा सल्यक्रिया गर्नु पर्छ:
> कुनै जटिलता देखिएमा (औषधीले दुखाई कम नहुने, पिसाबमा रगत अथवा पिप देखिए, ज्वरो आउने)
> मृगौला धेरै सुन्निएको (increasing hydronephrosis) अथवा मृगौला ले कम काम गरेको (Increasing Urea/Creatinine)
> एउटा मात्रै काम गर्ने मृगौला भएका बिरामी (Single functioning kidney) अथवा दुबै मृगौला मा पत्थरी भएको (bilateral ureteric stone)
> अस्पताल बाट निक्कै टाढा घर वा परेमा तुरुन्त स्वास्थ्य सेवा लिन नसकिने अवस्था भएको
> पत्थरी १ महिना सम्म न निस्किएर मृगौला बिग्रिदै जाने जोखिम भएको (Prolonged obstruction)

This 50-year young gentleman was referred from another hospital with a large 25x20mm triangular stone at the junction of right partial duplex ureter and multiple large left renal stones. He underwent two surgeries 2 days apart:
1. Right URSL with two DJ stents occupying both duplex system
2. Right URSL (to clear remaining fragments) followed by Left PNL
Total hospital stay: 6 days
