Do lumbar disc herniations require surgery?
Slip disc or lumbar disc herniation or pr*****ed intervertebral disc is one of the most common causes for low back and leg pain. The herniation is usually observed between the 4th and 5th lumbar vertebrae (L 4 L 5) followed by between the 5th Lumbar and 1st sacral vertebra (L5 S 1).
Diagnosis is based upon thorough clinical evaluation supplemented by Magnetic Resonance Imaging (MRI) of lower back.
A Meta-analysis of 11 cohort studies has shown that up to 70% of slip discs heal spontaneously over a period of 6 months. More than 90% of the patients experience pain relief with non-operative treatment only.
Our own study on lumbar disc herniation revealed that out of 2500 + patients with acute and chronic disc herniations seen over a period of last one year, less than 1% of the patients required surgical decompression which is contradicting the international reported incidence of as high as 5 to 20%.
Absolute surgical indication is the cauda equina syndrome when the patient experiences progressive muscles weakness and develops bowel / bladder dysfunction. The risk is 0.1%. Surgery may also be considered if non-operative measures have failed and exhausted. Surgery in appropriately selected patients fare an excellent outcome.
Approximately two-thirds of lumbar disc herniations resolve on their own, without surgery. Most people improve with time, rest, and conservative care like physical therapy or medication. The body is surprisingly good at healing disc injuries naturally.

Reckless motorbiking resulted in critical injuries to the 18 years old boy. His pillion rider got serious injuries and two of his friends died on the spot.
He was brought to the hospital in the middle of the night at the brink of his life & death and the crowd was huge. A dedicated medical team gave their best of their knowledge and skill. He had to undergo multiple surgeries.
He received a discharge after about three weeks of hospital admission. There was no CROWD, no MEDIA and no YOUTUBER to seize the moment of his returning back home. There was no one to disseminate the information to the public about his recovery and the endeavors put by our nurses, paramedics, hospital staffs and doctors for his holistic management.
As usual our joy of his recovery is unfathomable. We thanked each of the members of the medical team, shared happiness amongst ourselves and congratulated each other. Neither from the family or from anywhere we received appreciation.
However, we are happy and continue to do our job with the best of our knowledge. On your part please do not be NEGLIGENT in the care of your children. We wish you all very healthy life so that you never ever have to visit any of the hospitals.
