स्यांग्जा निवासी यी महिला माथि र तलको हड्डी को सन्तुलन नमिलेको कारण बोल्न र चपाउन गार्हो हुनु को साथ साथै आत्माबिस्वासको कमि महसुस हुनुको कारण उपचार को लागि हामी कहाँ आउनु भएको थियो । हामीले उनलाई सल्यक्रिया बाट उपचार गर्यौ। नया प्रविधि बाट उपचार गरेको हुनाले १ वर्षामा नै उनको उपचार सम्पन्न भयो। नभएको दांत उनले पोखरामा लगाउने निधो गरेकी छिन्।

A Life-Changing Surgery for a Young Girl from Dang.

We recently helped a 23-year-old girl from Dang who had been unable to open her mouth for her entire life due to a locked jaw joint (TMJ ankylosis). She had a fall when she was young but was unaware that she needed urgent treatment. The jaw got fused to the wrong location and got locked. This condition made eating, speaking, and even smiling extremely difficult, greatly affecting her daily life.

To restore her jaw function and improve her facial appearance, we ( A team of maxillo-facial surgeon and Orthodontist ) performed a custom-designed total jaw joint replacement (TJR). Ideally, a more complex Both-Jaw surgery would have been best, but due to her financial limitations, we chose a different approach. Along with the joint replacement, we also corrected her jaw alignment and chin position to improve her facial symmetry.

Now, she can eat, speak, and smile with confidence—a huge improvement in her quality of life after 15 years

People in rural area are generally unware of this kind of service and do not get treatment for a long time. When the jaw deformity is severe, jaw surgery is the only option for the best result. Jaw deformities are not life threatening condition but its treatment is of course a life changing procedure.

Severe gummy smile is best treated with surgery. However, mild to moderate cases can be treated with braces only. Braces are not only used for improving smile esthetics, it also helps to improve facial proportion. This lady had a gummy smile and also wanted to improve her lips and chin position. She denied surgery and was treated with braces and screws. The total duration was around 2 years.

समयमै उपचार गर्नु धेरै महत्त्वपूर्ण हुन्छ किनभने यसले भविष्यमा जटिल उपचारबाट जोगाउँछ र खर्चको पनि बचत गर्दछ। उदाहरणका लागि, यी महिलाले ७-८ वर्षको उमेरमा लडेको थिइन् र सम्भवतः चिउँडोमा चोट लागेको हुनु पर्छ। सल्लाह दिने कोही नभएकोले, चोट पटक सामान्य होला भनि उनलाई अस्पताल लगिएन। पीडा बिस्तारै कम भएता पनि उनको मुख खोल्ने समस्या आउन थाल्यो र उनको सम्झनामा, १० वर्षको उमेरसम्म उनको मुख लगभग खुल्न बन्द भैसकेको थियो। उमेरसंगै अनुहारको वृद्धि र विकास हुनमा TMJ जोर्नी सामान्य हुनु अत्यन्तै जरूरी हुन्छ। यहि कारणले गर्दा उनको तलको जबडा पनि राम्ररी वृद्धिविकास हुन सकेन। उपचार शुरु गर्दा उनी ४५ वर्षकी भइसकेकी थिइन्।
उनले ठीकसँग खान चपाउन असम्भव थियो र ब्रश गर्न पनि गाह्रो थियो। उनको मुखको सरसफाई खराब थियो र धेरै दाँत हल्लिएका थिए। परीक्षण गर्दा, बच्चा बेला लडेको कारण दुवै TMJ जोर्नी भाचिएर गलत ठाउमा (टाउकोको फेद संग) जोडिएको रहेछ। त्यसैले maxillofacial surgeon र Orthodontist को टोलिले दुवै तिरको कृत्रिम जोर्नी प्रत्यारोपण गरेर असामान्य जोर्नी र दाँतको टोकाई सचाइयो। कुल उपचार अवधि करिब १.५ वर्ष लग्यो।

करिब ३५ वर्ष पछि उनले बल्ल मुख खोल्न सक्ने भएकी छिन् । मज्जाले चपाउन सक्छिन र अहिले उनको मुस्कान फेरिएको छ।

नोट: अनुहारमा चोट पटक लागेमा बाहिर बाट गम्भीर समस्या नदेखेता पनि समयमा नै देखाउनु पर्दछ। समयमा नै उपचार गरे भविष्यमा जटिल उपचार गर्नु पर्दैन।

यस युवतीले काठमाडौ उपत्यकाको कुनै ठाउमा दांत नमिलेकोले braces उपचार गराएकी थिइन्, तर दुर्भाग्यवश उनी गलत व्यक्तिको हातमा परिन । उनको माथिका २ वटा दाँत निकालेर भित्र तानिएको रहेछ। Orthodontic उपचार गर्दा दाँत निकाल्ने प्रक्रिया एकदमै नियमित कुरा हो तर उपचारको पद्दति मिल्नु जरुरि छ। दुर्भग्यवस यो उपचारमा त्यो पद्दति नमिल्दा माथिको दांतहरुको जरा नराम्रो संग गलेको देखियो। दांतले माथिको हड्डीमा थिचेर हड्डी र दांत दुवै गलेको देखियो। यस्तो अवस्थामा दाँत निकालेको खाली ठाउँ बन्द गर्न गाह्रो हुनु स्वाभाविक हो। यस्तो अवस्था आउंदा, त्यहि समयमानै सुधार्नुपर्ने थियो, तर उपचारकर्ताले ठाउँ लुकाउन ब्रिज नै लगाइदिएका छन्। उनी अहिले ४ वर्षभन्दा बादी समय सम्म braces मा छिन्। ६ महिनामा विदेश जादै छिन् ।

नोट: अर्थोडोन्टिक उपचार संवेदनशील हुन्छ | गलत हातमा पर्दा braces को गलत उपचार पद्दति र समस्या लुकाउन ब्रिजको गलत दुरुपयोग जस्ता समस्याहरू उजागर गरेको छ ।

कुनै पनि उपचार गर्दा आफुलाई उपचार गर्ने व्यक्ति त्यस उपचार पद्दति संग सम्बंदित छन् कि छैन भन्ने कुरा ध्यान दिन एकदम जरुरी छ।

Temporal Mandibular Joint प्रत्यारोपण एक जटिल उपचार हो। उत्क्रिस्ट नतिजाको लागि यो म्याक्सिलोफेसियल सर्जन (maxillofacial surgeon) र ओर्थोडोन्टिस्टको (orthodontist) को संयुक्त प्रयासले गरिन्छ। उदाहरणको लागि ४८ वर्षीय यी महिला हाम्रो विभागमा बायाँ कानमा अत्याधिक पीडा हुनु को साथ साथै, मुख बांगो हुँदै गएको, मुख खोल्न गाह्रो हुँदै गएको, र माथि र तलको दाँतको सन्तुलन हराएर खाना चपाउन नसक्ने समस्याको साथ आएकी थिइन्। जाँचपछि पत्ता लाग्यो कि उनको पहिले गरिएको जोइन्ट प्रत्यारोपण टाउको को हड्डी संग खिएकोल अत्यधिक पिडा भएको, मुख बांगो हुँदै गएको र दाँतको सन्तुलन बिग्रेको थियो। यसैले हामीले उनको पुरानो प्रत्यारोपण हटाएर अत्याधुनिक प्रविधि बाट उहाको अनुकूल जोइन्ट प्रत्यारोपण गरियो र साथ साथै माथि र तल को दांत को सन्तुलन मिलाउन (Braces)ले उपचार गर्यौं, जसले उत्कृष्ट नतिजा हासिल गरियो।
अहिले अत्याधुनिक प्रबिधिबाट नेपालमै TMJ प्रत्यारोपण अन्तर्राष्ट्रिय मापदण्डअनुसार गरिन्छ।
