Would you consider your menstrual flow to be normal or heavy?

It's possible you may think you have a normal flow when actually your menstrual blood loss is higher than normal, or vice versa

it can be tricky to determine, so here are some characteristics to help self-identification of heavy menstrual bleeding (HMB)

if you have HMB, it should not be left untreated because
1. you don't need to suffer through heavy bleeding and pain every month. It's not normal
2. it is likely you'll have anemia and this can cause other problems like fatigue and shortness of breath. there are blood tests to check for this
3. there will be an underlying cause or medical condition for HMB and the sooner you get it checked the better

It matters because it helps to know what’s driving it so we know the best way to treat it

Not all PCOS is the same, and it can be a bit of a spectrum. So if one person has insulin resistance PCOS, another may have adrenal PCOS and the treatments would not be the same. You could also have two people with insulin resistance PCOS but one has gone untreated for a longer time which changes the treatment approach again.

In fact, you could be diagnosed without an ultrasound!

PCOS is a hormonal condition characterised by high androgens ("male hormones"). If you don't have high androgens, then it's not PCOS - even if you have polycystic ovaries (high amount of follicles)

Diagnostic criteria for PCOS includes 1 + 2 or 3 or all 3 below:

1) hyperandrogenism - high levels of testosterones, either biochemically (shown on a blood test) or clinically (visible with hirsutism, acne, hair loss).
2) oligo-anovulation - longer, irregular cycles and ovulation dysfunction
3) polycystic ovarian morphology - >20 follicles (egg sacs) per o***y, larger in size than normal

There are also other reasons why you could have androgen excess which should be ruled out first before PCOS is considered

So, if you have been told you have PCOS purely from an ultrasound and no other diagnostic criteria, it could be good to get a second opinion. Incorrect diagnosis can be harmful in many ways, especially when you google treatment for PCOS...

Conversely, if you've been told you don't have PCOS because your ovaries look normal, but you have the other two symptoms... I also recommend you get a second opinion

If you're not already tracking your cycle, can I ask why?

if you needed a few reasons why you should, here's 7!

tracking your cycle doesn't have to be complicated but it provides some really useful information for yourself to get to know your body, as well as your practitioner if you need some support with your hormones or fertility

if you're planning to conceive in the near future, or you want to avoid pregnancy, charting can help identify your fertile window to know when to get busy or use precautions!

it's also useful to track how regular your cycles are, if your bleed is heavy, medium or light, if you get spotting, and different types of cervical mucus (to help identify your fertile window.. more on this soon!)

you can also record your s*x life, and if you used protection or not which is useful for both conception and avoiding pregnancy

the journal feature is also great to record any symptoms you might be getting (see my previous reel 😉)

Have you experienced medical gaslighting?

I can vividly remember my GP laughing at me when I went to him and said there was something wrong with my hormones when I hadn’t had a period for over 90 days..

Or when I went to another GP recently and she told me that my cycle irregularity didn’t have anything to do with hormones 🤔

Both times I have had to push for bloods and testing, only to find my guesses were right.

If you are not being listened to, told that it’s just how it is, or it’s not what you think it is.. I highly encourage you to get a second opinion.

I know it can sometimes be intimidating to question or press issues with a medical professional. However, no one knows your body as well as you do. And if you know something isn’t right, it’s probably not.

Everyone deserves to be heard and have the care they need. Keep looking until you find it 🤍
