From 1st February, the government has enabled specially trained GPs and nurse practitioners to diagnose and prescribe medication for ADHD in adults.

Katikati Medical Centre will not be offering ADHD assessments. This decision is based on patient safety and our commitment to providing accurate diagnosis, safe prescribing and appropriate follow-up care.

Medical Specialists who provide ADHD assessments and treatment must have specific training and expertise in this area to ensure safe and effective care. This level of specialised assessment and management is not within the standard scope of general practice.

ADHD assessments require:
- An initial extended consultation of approximately 2 hours
- Additional follow-up appointments
- Administration time to process assessment tools and scoring

Unlike routine GP appointments, there is no additional funding for these services. As a result, the cost would be covered by the patient would be hundreds of dollars The extended time required would also reduce appointment availability for routine medical care for other patients.

If you are seeking an ADHD assessment, many providers accept self-referrals. When considering a provider, we recommend taking the following into consideration:

-Expected wait times and cost
- Their individual assessment and treatment processes
-Can the provider complete the assessment and initiate medication or do they only assess for ADHD and refer for treatment.

Our doctors generally do not start or titrate ADHD medications and prescribing responsibility transfers to the GP once medication is stable, the GP will then provide the ongoing scripts. Please note ADHD medication is not eligible for 12 month prescribing.

If you are unsure and would like further discussion, you can book a routine appointment with your GP who can refer you to a clinic of your choice.

IMPORTANT PRACTICE NEWS

We would like to let our community know about an upcoming change behind the scenes at Katikati Medical Centre.

From Monday 16th February, our team will be transitioning to a brand-new Practice Management System (PMS). This is the software we use every day to provide your care – including clinical notes, prescriptions, referrals, tests results etc. All general practices in New Zealand use a PMS, and while this change will not directly affect you as a patient, it is a significant upgrade for our team.

Learning a new system while continuing to care for our community is a big undertaking, and we appreciate your understanding as we work through this.

What does this mean for appointments? During the week of Monday 16 February, we will be operationing at reduced capacity while our team adjusts to the new system and ensures it is being used safely and effectively. The following week will also have slightly reduced capacity, though less than the first week. Please be assured that we will remain open for our usual hours during this time.

Temporary closure for IT changeover. This is to allow the IT team to complete all necessary work required before we open on Monday 16 February.

Friday 13 February: The practice will close early at 4pm

Saturday 14 and Sunday 15 February: The practice will be closed. Our weekend Acute Clinic (8.30am – 12pm) will not be running.

Patient Portal Update. As part of this change our current patient portal Health365 will no longer be available. We will be moving to a new patient portal called MyIndici. Patients affected by this change will be contacted by email prior to the changeover with clear instructions on what to expect and how to access the new portal.

In response to the recent ManageMyHealth security breach, MyIndici has supplied practices with a set of patient FAQs to help address questions about the MyIndici portal. If you would like a copy of these FAQs please contact reception.

12 MONTH SCRIPTS

From February 1st, GPs will be able to prescribe up to twelve months of medication at once.

It is likely that there WILL NOT be many patients where a 12 month script is appropriate, however for some a 6 month script MAY be suitable.

Be aware that a repeat prescription involves much more than just ticking a box and pressing send. Whilst doing a repeat prescription we check:

• Have you been seen in the practice or in hospital since your last visit or prescription request? Do we need to see you for follow-up?

• When did you last see us for a proper review of your medications? Do you actually need an appointment this time?

• Are the medications prescribed still appropriate for you, is the dose still correct, is this medication still best practice (things can change quickly in the medical field).

• Are your blood tests up-to-date? Eg deteriorating kidney function may mean doses of medications need to change.

• When was your blood pressure last done?

• Is your screening up up-to-date e.g. mammograms and cervical screening?

• You haven’t asked for a prescription for the last five months; are you actually taking your medications as prescribed?

• You have only asked for repeats for two out of your four medications; are you taking all of your medications as prescribed?

So virtual prescription reviews are a good safety net between appointments to ensure that your health is optimised. If we don’t look at your notes for 6-12 months this safety net is lost.

The government has said that GPs doing longer scripts will free up time to see more complex patients. This is not true. There is no capacity to see more patients because current funding does not provide for it. Scripts are already done in our lunch breaks and at the end of the day – doing less scripts wont free up extra appointment slots.

The government has said that longer scripts will save New Zealanders money. This is also not true. New Zealanders now understand that general practice is underfunded and at a crisis point. Practices charge a fee for the work involved, as explained above, to process a repeat prescription. General practice funding is so dire that the income received from repeat prescriptions is a significant part of their total income. If this income is lost, practices will need to raise their patient copayments for appointments, charge more for other services and create new charges for services that are currently free. If practices do not do this, they will need to cut services, make staff redundant or close completely, all measures that would result in worse patient care.

The government “consulted” on this with the Royal College of General Practitioners, GenPro (General Practice Owners Association) and the Treasury and ALL said this was not a sound idea. But it seems that the government really just wants a good political soundbite.

So be prepared to have a chat with your doctor about whether longer scripts are suitable for you (you CANNOT request a script longer than three months from reception). Don’t be upset if it’s deemed that they are not. And please don’t be surprised if you see prices increasing. This isn’t something we want but will be something we are forced into.

Christmas message

As we get closer to Christmas and the summer holiday period, Katikati Medical Centre wants to communicate with you about the increased pressures we face during this period. Each year, we work hard to provide quality care to our patients. However, during this time, we experience a significant surge in demand whilst also operating with reduced appointment availability as staff take some well deserved leave.

To help manage this period:

If you are due a medication review or a repeat prescription during the period of 15 December to 23 January 26, we ask that you book your appointment now for that time period, or that you send in your prescription request early so your doctor can action it. We encourage you to plan ahead for any non-urgent health concerns.

Our focus will be on providing essential medical services to our existing patients. Due to the high volume of patients we see, visitors can be seen only when capacity permits.

MEASLES

Alarmingly, measles cases are on the rise in New Zealand.

Measles is NOT HARMLESS. 1 out of every 1000 peole infected in developed countries like NZ will die. Common complications include ear infections, diarrhoea, and pneumonia. In New Zealand, 15 measles cases out of 100 are hospitalised. 1 in 1000 cases develop encephalitis (inflammation of the brain), of these 15% die and one third are left with permanent brain damage.

Measles can also increase the risk of complications in pregnancy - eg miscarriage and premature labour.

The risk of very serious disease is greatest for those under 5 and those over 20, and for anyone with a weakened immune system. Such people are often unable to be immunised and rely on protection from those around them being immunised.

Vaccination is the best way to prevent measles. The vaccine is safe and effective and FREE for anyone under 18 regardless of your immigration status, and for anyone else eligible for NZ funded healthcare.

PRACTICE MANAGER OF THE YEAR

Our utterly amazing Keren Hoogwerf has been named the PMAANZ (Practice Managers Association Australia and New Zealand) New Zealand’s practice manager of the year.

Keren joined our team over a decade ago as a receptionist. Her capability, initiative, and calm professionalism quickly stood out, and a few years later she moved into the role of Office Manager. Approximately four years ago, Keren stepped into the position of Practice Manager and she has excelled in this role ever since.

Keren is the linchpin of Katikati Medical Centre. She brings a spirit of kindness, compassion, and integrity to everything she does, and leads by example. Her ethos shapes the culture of our team and the experience of our patients.
