Posterior Cloaca is an Anore**al Malformation in which females are born with a single perineal or***ce where the a**s is normally located. The urethra and va**na are fused together, as a urogenital sinus, that is posteriorly deviated, and it opens in the anterior re**al wall.
When the a**s is normal, size and location, the recommended approach is a transanore**al (dividing the a**s and re**um in the midline) and doing a total urogenital mobilization to move urethra and va**na forward.

Patients with recto-perineal fistula or recto-vestibular fistula do not need a high pressure distal colostogram (when they have a colostomy) prior to the operation. It is known where the re**um is located.
Trying to do a PSARP in all other malformations without a properly done high pressure distal colostogram, is performing a blind surgical exploration, and it may lead to undesirable complications.

When do patients with anore**al malformation need their first colonoscopy?
At 45 years of age. For females with colonic neo-va**na, during the colonoscopy, neo-va**na also needs to be examined.
Any new onset of neo-va**na symptoms (bleeding, abnormal mucous production) will require examination.
If there is a family history of colonic cancer, the first colonoscopy should be at 40 years of age or 10 years before the age of colonic cancer onset in the family.

When a patient is born with cloacal exstrophy, a unified approach by urology and colore**al (+gynecology, when female) is essential.
After separation of the intestinal tract from the urinary tract, an end colostomy should be made, omphalocele closure should be attempted, as well as bladder closure if possible.
Around three years of age, we recommend starting trials of bowel management through the stoma to determine if the patient is a candidate for stoma pull-through.
