To all my patients: due to the COVID-19 global healthcare pandemic, many of you have missed your scheduled outpatient follow-up consult. For the safety of everyone, we were advised to close our clinics. I feel the apprehension of most of you, and I also worry about your general well-being and safety.
Starting next week, I will have my clinics, with observance of each hospital’s policies on re-starting clinics.
The clinics will be STRICTLY BY APPOINTMENT ONLY, for now. Kindly TEXT my secretaries for your appointment. There will be limit of patients per clinic slot, as allowed by the hospital’s clinic administrators.
Kindly observe the following:
1. Wear mask at all times. Observe hand hygiene, bring hand sanitizer/alcohol. Avoid touching your face.
2. One watcher/bantay for patients needing assistance (wheelchair-bound, elderly)
3. Observe social distancing. Observe silence. Bawal na po kwentuhan sa waiting area habang waiting for your turn.
4. If it is not your turn, stay inside your vehicle and wait for the secretary to text you
5. For NEW PATIENTS, kindly bring referral letter (if available ), list of meds and all available laboratory results (kahit ung luma)
6. Each hospital will be requiring all patients and accompanying persons to fill-up Health Declaration Form. Kindly fill up the form HONESTLY.
7. If you have symptoms : FEVER, COUGH, SHORTNESS OF BREATH and history of exposure to COVID-19 patients, please proceed to EMERGENCY ROOM for assessment
