TIME/VENUE: ROOM 107 9:00AM TO 11:30AM

Initial assessment on the following dates:

SEPTEMBER 2-3, 2025 - 9:00AM to 4:00PM

Pwede na pong humingi ng laboratory request at medical certificate form sa clinic mula ngayon.

Ang Pregnancy test (for females only) ay sa ating campus clinic po gagawin. Bumili lang ng pregnancy test kit. Kung kasama po sa package e within 1 week lang po ito valid. So ulitin na lang po ito sa Clinic.

Kung may mga katanungan po ay approach nyo lang po si nurse o message po sa page. Maraming salamat po ☺️

Time: BSE English 4A - 9:00 AM to 10:30 AM
BSE Filipino 4B - 10:30 AM to 12:00 NN
BEED 4C - 1:00 - 2:30 PM
BEED 4D - 2:30 - 4:00 PM

REMINDERS:
1. Please be on time.
2. Procedure/Steps:
- STEP 1: Kumuha ng number para sa pagkakasunud sunod
Pakikuha po ang inyong Medical/Dental Form kay nurse
bago kayo pumila. Pakibigay kay nurse yung mga
medical certificate o copy ng reseta kay nurse para
mai-attach sa inyong form
- STEP 2: Pumunta sa may lababo (sink) para mag gargle ng
Mouth wash solution sa loob ng isang minuto.
- STEP 3: Pumila sa Medical (Table 1) o Dental (Table 2) para sa
inyong evaluation.
- STEP 4: Kapag tapos na, ibalik ang Medical/Dental Form kay
Nurse. Pumirma sa monitoring list. Isauli ang inyong
number bago kayo umalis.

3. Kung kayo ay magkakaroon ng ubo o sipon ay mangyari po lamang na kumunsulta na agad sa pinakamalapit na Health Center o Municipal Health Office.

pa rin ang KLASE, all levels, public at private schools pati na rin ang trabaho sa lahat ng GOVERNMENT OFFICES sa buong Lalawigan ng Rizal bukas, July 24, 2025, dahil sa patuloy na pag-ulan dulot ng Bagyong Dante at Emong, at Habagat (Southwest Monsoon).

Bukas pa rin ang mga tanggapan na nagbibigay ng serbisyong medikal, disaster at emergency response at iba pang mga importanteng serbisyo.

Ang desisyon sa work suspension sa mga private companies ay nakadepende sa kanilang management.
