Thank you for your interest in Clinica Antipolo Hospital and Wellness Center, Inc. - Physical Medicine and Rehabilitation Department.

Before coming to the department for your schedule CONSULTATION, we humbly request that you fill out our HEALTH DECLARATION FORM as part of our pre-screening process. Make sure that you have pressed the "SUBMIT" button for your form to be received by our system.

If you are a card holder, we also request that you fill out our HMO INFORMATION SHEET for us to have a record of your HMO details for faster approval of your Consultation and PT sessions. This form will only need to be filled out once.
