The Importance of Decentralizing Specialized Cardiac Care

I’d like to share the story of one of my patients whom I saw at my clinic in Pototan.

He had been going back and forth to a medical facility for recurrent chest pain. Over time, the pain became crescendo in nature, but without access to a specialist nearby, he waited until he could see me at my clinic.

After careful history-taking and review of his ECG, it was clear that this was more than a simple anginal episode. I immediately requested a stat troponin, which confirmed our concern—the patient was already having a myocardial infarction.

From the clinic, we promptly arranged his transfer to a facility with a cardiac catheterization laboratory. An emergency coronary angiogram revealed critical proximal lesions in the LAD and a left-dominant LCX. He underwent immediate angioplasty and, thankfully, had an unremarkable recovery.

This case underscores a crucial reality: timely access to specialized cardiac care saves lives. Had our paths not crossed that day, this patient might never have lived to embrace his wife and child this Christmas season.

Decentralizing cardiac services—bringing expertise closer to where patients live—can make the difference between life and death.

Iferior vena cava (IVC) filter successfully deployed today — via the internal jugular approach!

First time wearing a lead gown again after more than a year 😅

Grateful to work side-by-side with a fellow Antiqueño, Dr. Jesrelle M. Zamora who deployed the device. Thank you for sharing your skills to our charity patients doc.

Our patient is a 54/F from Antique who came in with persistent vaginal bleeding and bilateral leg swelling. As the peripheral vascular specialist, I did her venous duplex and found a saddle-like thrombus in both iliac veins. Since anticoagulation wasn’t safe for her, I facilitated the transfer of the patient and recommended an IVC filter insertion to protect her from a life-threatening pulmonary embolism.

Moments like this remind me why decentralizing cardiac and vascular services matters.
