CAREGIVERS/MIDWIFE RATE

per 12hrs. Duty

a) Patient with NO contraptions attached – Php 800.00

b) Patient with NGT, PEG, IV, etc. – Php 900.00

c) Patient with Tracheal tube/Mechanical Ventilator – Php 1,000.00

PHYSICAL THERAPIST

Per session rate depends on the therapist

Our Private Medical Provider/Agency will submit the following

LEGAL REQUIREMENTS such as:

ü Updated Resume/CV

ü NBI clearance

ü PRC License for RN’s

ü NCII certificate for Caregivers

ü Transcript of Record (TOR)

ü Diploma

ü COE (Certificate of Employment)

ü Other credentials
