2.) Updated Personal Data Sheet (CS Form 212, Revised 2017) at http://www.csc.gov.ph
3.) Certified True Copy of Diploma and Transcript of Records
4.) Certified True Copy of Board Rating and updated PRC license
5.) Photocopy of Certificates of Trainings/Seminars relevant to the position
6.) Photocopy of Certificates of Employment and Clearance
7.) Photocopy of Clearance from former office (if previously employed)
8.) Neuro-psychiatric evaluation result (by BGHMC)
