If you are interested in applying, please note that your application letter must be addressed to: LARRY C. HAYA, JR., MD, DPCP, MBA, Medical Center Chief II.
Key Requirements Checklist:
1. Two (2) copies of Application letter.
2. Updated Personal Data Sheet (CS Form 212, Revised 2025), which can be downloaded at http://www.csc.gov.ph.
3. Certified True Copy of Diploma and Transcript of Records.
4. Certified True Copy of Board Rating and updated PRC license.
5. Photocopy of Certificates of Trainings/Seminars relevant to the position.
6. Photocopy of Certificates of Employment and Clearance.
7. Photocopy of Clearance from former office (if previously employed).
8. Neuro-psychiatric evaluation result (must be conducted by BGHMC).
9. For newly licensed physicians: PMA membership number and proof of application to BBMS.
