If you don't have a smart phone or application is glitching, you can personally visit the City Health Services Office's Front Desk to ask for assistance or go directly to the Sanitation Division at the 2nd floor.

Before paying your working permit and cedula at the 3rd floor, proceed first to the cashier at the ground floor to pay and accomplish requirements for the health certificate. You will be issued a working permit form by the Sanitation Division at the 2nd floor.

After accomplishing your health certificate, you may proceed to Room 303, Health Promotion Unit, 3rd floor, to pay for a cedula and/or working permit. Staff of the City Treasury is temporarily stationed there.
Make sure you have a valid ID indicating your Baguio Address. If you have no ID with a Baguio Address, go to your respective Barangay of residence, and they will issue you the documents.
If it is your first time to avail a Cedula, proceed directly to your respective Barangay Hall where you will be issued both Cedula and Working Permit.

You may avail of laboratory services at the City Health Services Office (Teodora Alonzo Street, or you may opt to avail at private facilities if your employer provides health insurance for the following required laboratories.

The following are the current laboratory fees:
a. Chest X-ray: PhP 100.00
b. Stool Examination: PhP 156.00
c. Health Certificate Fee: PhP 100.00
d. Working Permit Fee: PhP 50.00 (regular) and PhP 150.00
e. Cedula: Fee will depend on your gross annual income last year
f. Police Clearance: PhP 150.00 to PhP 180.00 (fee will depend on transaction fee with third party)

๐Step 1: Proceed to the Front Desk of the City Health Services Office, T. Alonzo Street, you will be issued an application form.
๐Step 2: Proceed to the basement for laboratory works
๐Step 3: Go up again to the ground floor cashier to pay for the fees.
๐Step 4: Proceed to the doctor (Rm 104) for evaluation.
๐Step 5: Proceed to the Sanitation Division to schedule food handlers seminar and issuance of Health Certificate
