Types of Fever and How to Interpret Fever Clinically

Overview
Fever is a regulated rise in body temperature controlled by the hypothalamus. It reflects infection, inflammation, or immune activation, not just excess heat. Correct interpretation depends on fever pattern, duration, and associated symptoms rather than a single temperature reading.

Types of Fever
• Continuous, remittent, intermittent, relapsing, and step ladder patterns
• Each pattern provides diagnostic clues to underlying conditions such as malaria, typhoid, tuberculosis, or septic states

Clinical Interpretation
• Low grade, moderate, high grade, and hyperpyrexia indicate severity
• Hyperpyrexia above 41 °C is a medical emergency
• Always correlate clinical findings with laboratory results

Laboratory Approach
• No single test diagnoses fever
• CBC, CRP, ESR, blood cultures (before antibiotics), and targeted tests help identify the cause

Key Point
Understanding fever patterns improves diagnosis and prevents missed or delayed treatment.
