PNA congratulates this year’s International Council of Nurses’ Kim Mo Im Policy Innovation and Impact Award recipient, Hon. Dr. Leah Samaco-Paquiz, RN of the Professional Regulatory Board of Nursing of the Professional Regulation Commission.

She will be awarded during the International Council of Nurses Congress on July 1-5, 2023 in Montreal, Canada.

Follow the steps below:

Step 1- Visit PRC LERIS website
Step 2 - Log in
Step 3 - Select transaction
Step 4 - Click Oath
- Fill up Oath Type: FACE TO FACE MASS OATH TAKING
- Fill Up Profession: NURSE
- Application number: YOUR NOA
Step 5 - Choose REGION IV-A
Step 6 - Choose QUEEN MARGARETTE HOTEL, BRGY. DOMOIT, DIVERSION ROAD, LUCENA CITY
Step 7 - Submit application
Step 8 - Download oath taking form
Step 9 - Print and fill in the form (make sure to print the one with the QR code)
Step 10 - Bring it on Feb 3, 2022
