1. 4 PCS PASSPORT SIZE PICTURE WITH COLLAR, WHITE BACKGROUND AND NAME TAG
2. CERTIFICATE OF EMPLOYMENT
a. Notarized, certificate with mobile number and Barangay Certificate certifying that you worked as a Caregiver of the person issuing COE.
3. 1-VALID ID ( 3 photocopy)
4. Covid19 Awareness certificate
5. TOP (Tesda Online Program, Elderly Care)
