• Patient’s name, address, and health record number
• Generic medicine name (e.g., Fenofibrate 160mg/tablet)
• Clear dosage, frequency, and duration (1 tablet daily for 1 month)
• Prescriber’s signature and license number
As per the Generics Act of 1988 (RA No. 6675), always use generic names in prescriptions, with the brand name optional.
Follow the “Right Patient, Right Medication, Right Dose, Right Route, Right Time, Right Reason, Right Documentation” guidelines.
Common prescription errors include:
• Incorrect Drug Selection: Prescribing the wrong medication due to similar names or confusion.
• Wrong Dosage: Over- or under-dosing, often from miscalculations or unclear instructions.
• Frequency Errors: Incorrect timing or frequency of administration (e.g., daily vs. twice daily).
