FOR 5 YEARS OLD AND ABOVE
*1 valid ID ( If no valid id available, bring original Birth Certificate. For married women who wants to use the surname of their husband, bring your certificate of Marriage.)
FOR 1-4 YEARS OLD
*Birth Certificate of the Child (original).
*National ID (card, paper or digital) of the accompanying parent/guardian.
