World Patient Safety Day 2025 | September 17
Theme: Safe Care for Every Newborn and Every Child

On September 17, 2025, the world comes together to observe World Patient Safety Day, a global movement to improve patient safety and reduce preventable harm in healthcare. This year’s theme, “Safe Care for Every Newborn and Every Child,” highlights the critical need to protect our youngest and most vulnerable patients—newborns and children—in healthcare settings.

Newborns and children face unique risks in medical care, from medication errors to infections and diagnostic challenges. The good news? Many of these incidents are preventable with the right strategies, policies, and collaboration. Join us in raising awareness and taking action to ensure every child receives the safe, high-quality care they deserve!

Why this theme matters:
• Higher risks for kids: Newborns and children are especially vulnerable to harm in healthcare settings, including intensive care, surgeries, and medication administration.
• Preventable harm: Studies show a significant portion of adverse events in paediatric and neonatal care can be avoided with proper interventions.
• Lifelong impact: Safe care in early years lays the foundation for healthier lives, reducing long-term health complications.
• Global challenge: Medication errors, healthcare-associated infections, and diagnostic mistakes remain major contributors to preventable harm worldwide.

What we’re doing for World Patient Safety Day 2025:
✅ Raising awareness: Highlighting the unique risks and needs of children in healthcare, as emphasized by the World Health Organization (WHO).
✅ Promoting solutions: Showcasing proven strategies and interventions to reduce harm in paediatric and neonatal care.
✅ Mobilizing globally: Lighting up iconic monuments and landmarks in orange to symbolize our shared commitment to safer care for kids.
✅ Advocating for change: Pushing for policies and practices that prioritize patient safety in paediatric healthcare systems.
✅ Engaging families: Recognizing the vital role of patients and families in improving safety, as championed by the World Patients Alliance.
✅ Encouraging collaboration: Bringing together healthcare professionals, policymakers, patients, families, and civil society to drive meaningful change.

How you can get involved:
• Share this post to spread the word about World Patient Safety Day 2025!
• Talk to your healthcare providers about patient safety in paediatric and neonatal care.
• Advocate for stronger safety policies in your community’s healthcare systems.
• Look for orange-lit landmarks in your area and share photos to show your support!
• Engage with organizations like the World Patients Alliance to learn more about patient-centered care.

Together, we can create a safer healthcare environment for every newborn and every child. Let’s make patient safety a priority—not just on September 17, but every day! 🌟

REMINDER TO ALL DOCTORS
Let’s prioritize patient safety with proper prescribing practices! Check out this sample prescription from Region II Trauma and Medical Center, highlighting key details:
• Patient’s name, address, and health record number
• Generic medicine name (e.g., Fenofibrate 160mg/tablet)
• Clear dosage, frequency, and duration (1 tablet daily for 1 month)
• Prescriber’s signature and license number

As per the Generics Act of 1988 (RA No. 6675), always use generic names in prescriptions, with the brand name optional.
Follow the “Right Patient, Right Medication, Right Dose, Right Route, Right Time, Right Reason, Right Documentation” guidelines.

Common prescription errors include:
• Incorrect Drug Selection: Prescribing the wrong medication due to similar names or confusion.
• Wrong Dosage: Over- or under-dosing, often from miscalculations or unclear instructions.
• Frequency Errors: Incorrect timing or frequency of administration (e.g., daily vs. twice daily).
• Route Mistakes: Administering a drug via the wrong method (e.g., oral vs. intravenous).
• Allergy Oversight: Failing to check for patient allergies.
• Illegible Handwriting: Unclear prescriptions leading to misinterpretation.
• Omitted Information: Missing patient details, duration, or prescriber details.
• Drug Interactions: Not considering interactions with other medications.

Always double-check prescriptions to ensure patient safety!

Stay vigilant and keep our patients safe! 💙

Understanding Diagnostic Error – Let’s Make Patient Safety a Priority!

Did you know that diagnostic error is defined as the failure to establish a correct and timely explanation of a patient’s health problem? This can include delayed, incorrect, or missed diagnoses, as well as failure to communicate the diagnosis to the patient.

Key Facts from Our Insights:
• 63% of diagnostic failures stem from patient assessment issues, such as the failure to establish a differential diagnosis, delays in ordering diagnostic tests, or inadequate assessments that miss critical signs like abnormal vital signs.
• 23% are linked to patient factors, including non-compliance with follow-up appointments or treatment regimens.
• 65% of ED diagnosis-related malpractice claims cite inadequate assessment, highlighting the need for thorough and timely evaluations.

World Patient Safety Day – September 17, 2024 emphasized the theme Improving Diagnosis for Patient Safety. The data reminds us that errors can occur at any stage— from initial assessment to communication with providers. Poor professional relationships or failure to read medical records can further complicate the process, making clear communication and detailed documentation essential.

Sharing with you some insights from a rich learning session with the Patient Safety Movement Foundation’s mentors and fellows, organized by Dr. Peter Lachman, which brought together fellows from different cohorts for a rich learnning experience.

Key Takeaways from Dr. Anurhada Pichumani’s Presentation
1 Leadership Journey and Local Impact:
◦ Dr. Pichumani’s transition from clinician to a global leader at ISQua (International Society for Quality in Health Care) highlights the power of leadership in driving patient safety. Her emphasis on “every global movement begins with a local moment” resonates as a call to action for grassroots efforts, especially in low- and middle-income countries (LMICs). -burgo

2 Strategies for Low-Resource Settings:
◦ Her focus on achieving significant outcomes in low-resource settings through good leadership, teamwork, local adaptation, standardization (e.g., accreditations), and data-driven improvement projects is particularly relevant for LMICs. This underscores the importance of contextualized metrics and capacity building, which are actionable even with limited technology or resources.

3 ISQua White Paper: Safe Care is the Right Care:
◦ The White Paper offers a practical framework for hospital-level patient safety improvements, adaptable to resource-constrained environments. The matrix you mentioned serves as a functional checklist to assess institutional strengths, weaknesses, and opportunities, guiding future directions without requiring advanced technology.
Observations and Insights
• Practicality and Adaptability: The emphasis on hospital-level strategies that don’t rely on national-level interventions or high-end technology makes the ISQua framework highly applicable across diverse settings. This aligns with Dr. Pichumani’s message about leveraging local strengths and opportunities, as highlighted in the Global Patient Safety Report’s performance scores.
• Leadership and Teamwork: Dr. Pichumani’s leadership journey and the focus on team effort reflect a universal principle: sustainable change starts with committed individuals and collaborative systems, even in challenging environments.
• Actionable Tools: The matrix is a standout feature, offering a structured way to evaluate and prioritize patient safety initiatives. This tool seems particularly valuable for identifying gaps and setting measurable goals, which is critical for data-driven improvements.
