Nearly 100,000 cases V-Fib or pVT cardiac arrest occur in the U.S. each year.

However, if you've run enough of these calls you've experienced refractory V-Fib/pVT, where the standard A-L pads and maximum energy settings don't convert the patient to a perfusing rhythm.

Dr. Sheldon Cheskes and his team in Toronto published a practice changing study this month in the The New England Journal of Medicine that demonstrated a significant survival improvement from Refractory V-Fib using Double Sequential External Defibrillation (DSED) and Vector Change (VC) defibrillation.

What did the study look at?
➡ 405 OHCA arrests 18 years old or older, with refractory ventricular fibrillation (or pVT).
➡ The first 3 defibrillation attempts used standard A-L pads.
➡ Patients were then cluster randomized to one of 3 groups:
➊ Continued standard defibrillation (136 patients)
➋ Vector change (144 patients)
➌ DSED (125 patients)

What were the results?
➼ Termination of ventricular fibrillation:
⦿ 105 patients (84.0%) in the DSED group (aRR 1.25)
⦿ 115 patients (79.9%) in the VC group (aRR 1.18)
⦿ 92 patients (67.6%) in the standard group

➼ ROSC occurred in:
⦿ 58 patients (46.4%) in the DSED group (aRR 1.72)
⦿ 51 (35.4%) in the VC group (aRR 1.39)
⦿ 36 (26.5%) in the standard group

➼ Survival with good neurological outcome:
⦿ 38 patients (30.4%) in the DSED group (aRR 2.21)
⦿ 31 (21.7%) in the VC group (aRR 1.71)
⦿ 18 patients (13.3%) in the standard group

What are the takeaways here?
➡ DSED more than doubled neuro-intact survival in rVF and we feel it should be the standard of care.
➡ VC Defibrillation (AP pad placement) is also an excellent alternative in these patients.
➡ Monitor manufacturers should work on creating a DSED/VC option using a single monitor (with 4 pads).
