Medical/Dental Requirements – BulSU Meneses Campus

To all incoming freshmen qualifiers, please be guided by the following health requirements as part of your enrollment process at Bulacan State University – Meneses Campus:

🔹 Health Requirements to Submit:

📄 Photocopy of Chest X-ray Result

📝 Printed Medical and Dental Health Assessment Form

✅ Medical Clearance (if applicable)

✅ Photocopy of PWD ID (if applicable)

🔹 NEW REQUIREMENT from BulSU Meneses Campus Health Services:

All enrollees are required to fill out an online Health Information Google Form

You must present this screenshot to the nurse on duty on the day of your Medical and Dental Assessment as proof of completion.
