The Department of Health Center for Health Development Caraga is now accepting qualified professionals to fill-in the following vacant position (PS-Contractual) under the National Health Workforce Support System (NHWSS):
Medical Technologist II - SG15
Area of Assignment: Butuan City
Midwife II - SG11
Area of Assignment: Butuan City

2. Four (4) copies of duly accomplished Personal Data Sheet form with recent passport size picture (CS Form no. 212, Revised 2017) which can be downloaded at www.csc.gov.ph;
3. Four (4) copies of duly accomplished Work Experience Sheet Form (CS Form No. 212 attachment, Revised 2017) which can be downloaded at www.csc.gov.ph;
4. One (1) copy of Transcript of Records and Diploma;
5. One (1) copy of License (RA 1080) including the Board Rating;
6. Copy of relevant training certificates;
7. Certificate as fellow of the relevant society and/or diploma of the relevant specialty society board;
8. IPCR Rating equivalent to one rating period (applicable to government employees only)
The deadline for submission will be on August 22, 2025.
