There will be covid-19 vaccination on NOV 5 (friday) at CUMC for 12-17 years old A3 category (with COMORBIDITIES).
For parents/guardians of my patients who are interested to register- please send name of your child to my secretary (0945) 894 3721.
She will forward the LINK for ONLINE REGISTRATION.

Please have the following ready:
1. Birth certicate
2. Valid ID of parent
3. Medical certificate

I will issue MEDICAL CERTIFICATE only to patients who already have existing records in my clinic.
