"Walang gamot sa Hepa"
"Pag may hepa ka, habambuhay ka nang may hepa"

Yan ang common nating marinig. Pero alam nyo ba na ito ay mali?

Ang mga buntis ay ini-screen para sa Hepatitis B infection sa first trimester. Ito ay nakukuha sa direct blood contact at pakikipagtalik. Maaaring mahawa ng baby habang buntis ang kanyang ina dahil ang Hepatitis B virus ay tumatawid sa placenta (Vertical transmission)

Kung nonreactive o negative, Hepatitis B vaccination is given.

Kung mag positive o reactive sa HbsAg screening, ang mga sumusunod ay nangangailangan ng baseline

-Liver panel
-HBV DNA
-HBeAg and anti-HBe

Ideally, ang HBV DNA at liver panel ay inuulit pagdating ng end of second trimester (26 to 28 weeks)

Kung ang HBV DNA ay >200 000 IU/ml, inooffer ang antiviral therapy sa mother at pagkapanganak ay tuturukan ng HepB vaccine at HepB Immunoglobulin si baby, ideally within 12 hours of life.

So ano ang gamot para sa Hepatitis B Infection during pregnancy more than 28 weeks AOG???

Ang gamot ay ang first-line agent na Tenofovir Disoproxil Fumarate na Pregnancy Category B (Safe na gamot during pregnancy)

Sharing with you the latest 2024 Childhood Immunization Schedule in the Philippines.

At the age of 1, your baby should have atleast received the following vaccines and most of them are available at local health centers and are given for free. These are the following:

BCG x 1 dose
Hep B x 3 - 4 doses
Diphtheria, Tetanus, Pertussis x 3 - 4 doses
Inactivated polio (IPV) x 2 - 4 doses
Oral polio (OPV) x 3 doses
Hib x 3 - 4 doses
Pneumonia x 3 - 4 doses
Measles x 1 dose
MMR x 1 - 2 doses

Other vaccines that may not be available at LHC but available in private clinics/hospitals and are of equal importance are the following:

Rotavirus x 2 - 3 doses
Flu x 2 doses
Inactivated Hepa A x 1 dose (*2nd dose can be given at a minimum interval of 6 mos from the 1st dose)
Varicella x 1 dose (*2nd dose is usually given at 4-6 years of age but maybe given at an earlier interval of 4 weeks to 3 mos from the 1st dose and is considered valid)
Japanese Encephalitis x 1 dose (2nd/booster dose is given 12-24 mos from the 1st dose)
Meningococcal x 1 - 3 doses

Ano ang BPS ultrasound?

Ito ang ultrasound na recommended para sa mga buntis na 28 weeks AOG at pataas. May mga high risk patients rin na 26 weeks palang for BPS ultrasound na. Ito ang ultrasound na magsasabi kung gaano ka-okay si baby.

Chinecheck dito ang apat na parameter para masabi na okay si baby...

1. Fetal breathing
2. Fetal tone
3. Fetal movement
4. Amniotic fluid index

Bawat parameter ay tig 2 ang score kaya ang perfect score ni baby ay 8/8 sa BPS ultrasound.

Ang complete BPS score is 10/10.

Saan manggagaling yung isa pang 2 points if apat lang na variable/parameter ang nakikita sa ultrasound?
.. sa Non Stress Test.

Ang NST kung reactive ay another score of 2 kaya magiging BPS 10/10. Ako, pag sinabing NST, oxgygenation lang ang iniisip ko. Kasi dito nakikita kung gaano ka-okay ang oxygen supply ni baby. Malalaman ito base sa kanyang heart rate pattern.

Sa susunod ididiscuss ko ng simple ang Non Stress Test.

1st month: mukha kang multo na black and white pero alam niya ang amoy mo

2nd month: pixelated version ka na pero black and white pa rin

3rd month: nagkakakulay ka na pero blurry ka pa rin

4th month: kitang kita ka na niya

Wag ka mag-alala, kahit ganyan ka sa paningin nya, alam nya na ikaw ang mama nya at nakikilala ka nya sa pamamagitan ng amoy at boses mo.
