May pagbabago po sa iskedyul ng bigayan ng gamot tuwing LUNES HANGGANG MIYERKULES na po ALA-UNA ng hapon (1PM).

Paalala po muli, ang pagkuha po ng gamot ay kinakailangan na may UPDATED na reseta.

Kung luma po ang reseta kailangan po muna magpakonsulta muli para malaman po natin kung parehong gamot pa din at parehong dosage pa din ang inyong iinumin.

Mangyari po na magmessage muna sa hotline 0963 903 9283 o sa FB Page City of Calamba Mental Health Program para sa pagkuha ng gamot upang di po masayang ang inyong pagpunta.

Ang City of Calamba- Mental Health Program ay nagbibigay ng LIBRENG serbisyo para sa Mental Health, ang mga ito ay ang mga sumusunod

1.LIBRENG Consulta/Assessment ng kalusugang pangkaisipan (MENTAL HEALTH) (BY Appointment)
2.LIBRENG Psychosocial Services (BY Appointment)
3.LIBRENG Gamot (Psychotropic Medications)
4.Mental Health at Psychosocial support
5.Assessment gamit ang MHGAP

Ang programa ay ACCREDITED ng Philhealth Outpatient Benefits Package

May LIBRENG snacks at tubig din na ibinibigay, komportableng upuan habang nanonood ng mga bidyo na makapagpapayabong ng kaalaman sa mental health habang naghihintay ang mga kliyente.

Ang mga serbisyong nakapaloob sa programa ay BUKAS PARA SA LAHAT maging taga ibang lugar ay maaaring maka avail ng aming mga serbisyo.

Para sa mga taong nangangailangan ng gamot para sa kanilang mental health condition, dalhin ang bagong reseta sa City Health Office, Brgy. 7 Calamba City upang makakuha ng gamot.
PAALALA: ang araw ng pagkuha ng gamot ay lunes, miyerkules at huwebes, ala-una ng hapon.

Ang City of Calamba-Mental Health Program ay may Philhealth Outpatient Package for Mental Health. Para sa konsultasyon ng iyong mental health, magpadala lamang ng mensahe sa page ng City of Calamba Mental Health Program at maari din sa 0963 903 9283. Ang bawat konsultasyon po ay by appointment.

📢 MAHALAGANG PAALALA PARA SA MGA PWD ID HOLDERS

Kung ang iyong PWD ID ay hindi makita sa DOH Registry, narito ang mga hakbang na dapat gawin:

1. I-verify ang Iyong Detalye:
Bisitahin ang https://pwd.doh.gov.ph/home.php at gamitin ang "ID Verification" tab. Siguraduhing tama ang format ng ID number. Makipag-ugnayan sa inyong LGU para sa tamang paraan ng pag-input.

2️. Kung lumabas ay “NO RECORDS FOUND,” Makipag-ugnayan sa PDAO:
Pumunta sa Persons with Disability Affairs Office (PDAO) ng inyong lugar. Sila ang makakatulong upang beripikahin kung nakarehistro na ang iyong ID sa system.

⚠️ Babala mula sa DOH:
Ang pag-gamit ng pekeng PWD ID ay isang krimen na pinaparusahan sa ilalim ng RA 7277 Section 46. Ang mga lalabag ay maaaring pagmultahin o makulong.
