Methemoglobinemia is a condition with life-threatening potential in which diminution of O2-carrying capacity of circulating Hb occurs due to conversion of some or all 4 iron species from the Fe2+ state to Fe3+ state. Fe3+ is unable to bind & transport oxygen leading to anaemia.

WBC DIFFERENTIAL

On consult rounds last week, I asked a resident about a patient's WBC differential and they stated reassuringly that there was a "neutrophil predominance".

I asked what they meant by the term.

"98% of the WBCs are neutrophils" was the response.

We looked at the total WBC together and it was 0.1 x 10^9/L.

98% of 0.1 (rounds to 0.1) = profound neutropenia!

This is an example of how misleading %'s are in hematology. Another e.g. is shown in graphic.

Forget the % and focus on absolute counts for the 5 WBC subtypes!
