2. Senior Citizen ID (para sa mga senior citizen)
3. PhilHealth ID (KUNG MAYROON)
4. Magdala ng proof of comorbidity tulad ng medical certificate mula sa iyong doktor na inissue sa loob ng nakalipas na 18 buwan; medicine prescription sa nakalipas na anim na buwan; hospital records tulad ng discharge summary at medical abstract o; surgical records at pathology reports.
