Nais pong ipaalam ng Pamunuan ng Notre Dame Hospital na simula sa Pebrero 1, 2023, lahat po ay pinaaalalahanan na mangyaring dalahin ang inyong “Card ng Bakuna” para sa CoviD – 19 na nagpapatunay na kompleto at may isa o dalawang “booster shots” ng bakuna kayong tinanggap.
Mangyari pong ipakita and nasabing “Card ng Bakuna” upang kayo ay mapayagang makapasok sa pasilidad ng hospital.
Maraming salamat po sa inyong pang – unawa at kooperasyon! Walang Card ng Bakuna! Di po pwedeng pumasok!
