TO: ALL INCOMING 1ST YEAR STUDENTS

>>> MEDICAL REQUIREMENTS

FOR BSHM & BTLED:
1. CBC (recent result)
2. CHEST X-RAY
3. DRUG TEST (within 6 months)
4. STOOL EXAM (recent result)

FOR OTHER COURSES:
1. CBC (recent result)
2. CHEST X-RAY
3. DRUG TEST (within 6 months)
>>Provide the ORIGINAL & PHOTOCOPIES in short bond paper.
(Do not include the X-ray Film)

R E M I N D E R S
1. Ask for a REFERRAL SLIP for your laboratory procedures in the
clinic.
2. PHYSICAL EXAMINATION is done ONLY in the clinic.
3. MEDICAL CERTIFICATE is certified ONLY by our School Physician.
4. For DRUG TESTING, bring any ID. The laboratory will require it.
5. Make sure that the Laboratories are DOH ACCREDITED.

All clinic transactions from July 28 to August 1, 2025 will be held at the Student Activity Center (SAC). Operating hours are 8:00 AM to 5:00 PM each day.

To all students planning to complete their laboratory procedures at Elishama Medical Lab: Please be advised that the lab will be closed on July 24 celebration of their Annual Fiesta in Compostela. They will reopen on Friday and Saturday ,July 25 and 26 but from 8:00 AM to 12:00 PM only.
