As of March 1, 2026, the online registration module in the old My PhilHealth Portal is no longer available; use the eGov App, Member Portal, or visit an office.

PHILHEALTH YAKAP
STEPS TO REGISTER FOR PHILHEALTH YAKAP
( UNSAON PAG ENROLL SA USA KA YAKAP CLINIC PROVIDER )

1. Select a Provider: Choose your preferred accredited YAKAP Clinic (formerly Konsulta package provider).

2. Registration Methods:
• eGovPH App: Download the app, create an account, find the PhilHealth section, and select "YAKAP".
• PhilHealth Member Portal: Log in and register online.
• Local Office (LHIO): Visit the nearest PhilHealth office for assisted registration, bringing a valid ID and filling out the PhilHealth Konsulta Registration Form (PKRF).

3. Validate Information: Provide your PhilHealth Identification Number (PIN) to finalize registration.

4. First Patient Encounter (FPE): Visit your chosen YAKAP Clinic for a medical assessment.

Important Notes:
1. Transferring Clinics: If you have already used your FPE, you can only request a transfer during the fourth quarter (October to December), effective January 1 of the following year.
2. Requirements: A valid ID is required for verification.
3. Checklist: Ensure your membership data is updated before registration.

Mahalagang piliin nang mabuti ang healthcare provider na mag-aalaga sa iyong kalusugan.

Ang kalusugan ay isa sa pinakamahalagang yaman ng tao. Upang mapanatili itong maayos, mahalagang pumili ng tamang health care provider o tagapag-alaga ng kalusugan. Ang tamang provider ay hindi lamang may sapat na kaalaman, kundi may malasakit at maaasahan sa pangangalaga sa pasyente.
