Patient: Why can’t I eat? Won’t I be hungry when I wake up? Am I not even allowed to drink water?

Dietitian: There are procedures where it’s best for our patients to be on “NPO,” or nothing by mouth (meaning no food or drinks), because it’s medically necessary.

"TYPE 5 DIABETES MELLITUS" is a newly recognized form of diabetes linked to *childhood malnutrition*, particularly protein-energy malnutrition. It was officially acknowledged by the International Diabetes Federation (IDF) in 2025 after decades of misclassification⁽¹⁾⁽²⁾.

Unlike type 1 (autoimmune) or type 2 (lifestyle-related) diabetes, type 5 diabetes—also known as *malnutrition-related diabetes mellitus (MRDM)* —is caused by impaired pancreatic development due to undernutrition in early life. This leads to *severe insulin deficiency* without the insulin resistance seen in type 2⁽²⁾.

It primarily affects *lean adolescents and young adults* in low- and middle-income regions of Asia, Africa, and Latin America. Many patients were previously misdiagnosed with type 1 diabetes, which can be dangerous since standard insulin doses may cause life-threatening hypoglycemia⁽³⁾.

Researchers estimate that *20 to 25 million people* worldwide may be living with this condition⁽²⁾. Treatment often involves *low-dose insulin or oral medications*, and early diagnosis is crucial to avoid complications.
