ATTENTION TO ALL & PLEASE BE GUIDED.

- HEALTH CARD ISSUANCE/RENEWAL AT GAISANO ILLUSTRE 5TH FLOOR WILL END AT MARCH 31,2023. PLEASE RENEW/PROCESS YOUR HEALTH CARDS EARLIER THAN THE SAID DATE.

- ONLY WALK IN CLIENTS ARE ACCEPTED IN GAISANO ILLUSTRE; BULK PROCESSING OF HEALTH CARDS ARE CATERED IN THE CITY HEALTH MAIN OFFICE LOCATED AT MAGALLANES.

- REQUIREMENTS ARE POSTED IN THE COMMENT SECTION. CLIENTS ARE EXPECTED TO HAVE THEIR COMPLETE REQUIREMENTS TO AVOID DELAY AND HASSLE ON PROCESSING THEIR HEALTH CARDS.
