5 TO 11 Y/O VACCINATION UPDATE

Sa tanang ginikanang buot pabakunahan ang ilahang anak edad 5-11 years old, pwede na magpa-lista sa Southern Philippines Medical Center (SPMC) 5-11 Y/O RESBAKUNA KIDS karong umaabot nga February 23-24 ug March 3-4.

Alang sa mga buot magpabakuna, palihog sunda kining mga lakang:

STEP 1: I-rehistro ang inyong mga anak sa DQR pinaagi sa: https://profiles.safe-davao.com. Mao kini ang requirements sa pag-register sa DQR: PSA Copy sa Birth Certificate o Valid Student ID.

STEP 2: Magparegister sa SPMC Resbakuna Kids pinaagi ani nga form: bit.ly/SPMCVaccination5to11. Siguroha nga ang inyong mga anak anaa sa saktong edad nga 5-11 years old sa maong vaccination day. Palihog gamit og usa ka form matag usa ka bata.

STEP 3: Hulata ang email confirmation 1-2 ka adlaw human sa pag rehistro.

STEP 4: Kung madawat na ang email confirmation, dalaha kini lakip sa uban pang mga requirements sa SPMC sa adlaw sa pagpabakuna:

- PSA Copy sa Birth Certificate o Valid Student ID sa bata
- Valid ID sa ginikanan o guardian
- DQR sa bata
- Ballpen
