The patient presented with marginal staining on an existing anterior restoration and initially expressed interest in direct composite veneers. After a thorough clinical evaluation, it was determined that replacing the existing restoration would be a more appropriate and conservative approach. With proper beveling and polishing techniques, the esthetic concern can be effectively managed without the need for veneers, which may not be indicated in cases where minimal intervention can achieve the desired outcome.

At the time of treatment, some areas of the teeth appeared slightly dehydrated, temporarily affecting shade perception. This is a normal clinical occurrence and is expected to resolve as the teeth rehydrate, resulting in a more natural and consistent appearance over time.

Our patient initially wanted direct composite veneers for the lower teeth, but after assessing the bite, we decided to take a more conservative and suitable approach. Instead, we proceeded with teeth whitening—and the results brought the lower teeth much closer in shade to the existing upper composite veneers.

Next step: diastema closure on the lower teeth.
Braces were not part of the patient’s preferred options, so we tailored the treatment plan to match both function and esthetics.

A patient presented with a chief complaint of a fractured, carious tooth ( #12 – right lateral incisor). Upon periapical radiographic evaluation, it was revealed that all upper central and lateral incisors were affected by infection extending to the pulp, indicating the need for comprehensive root canal treatment.

Initial management included oral prophylaxis with subgingival scaling to address periodontal health. This was followed by root canal therapy for the affected teeth. For the fractured tooth, a fiber dowel post and crown lengthening procedure were performed. Additional procedures included gingivectomy and placement of jacket crowns.

While the current jacket crowns may not offer the highest level of aesthetic refinement, they were selected based on the patient’s present needs and financial considerations. These restorations can be upgraded in the future to achieve a more natural appearance.

Most importantly, the patient expressed satisfaction and confidence in her restored smile—an outcome that remains our primary goal in every case.

A case of tooth discoloration after root canal treatment.

The ideal plan was internal bleaching followed by a jacket crown to restore the color and protect the tooth from possible fracture. However, due to time constraints, the patient chose direct composite veneers instead.

While veneers can greatly improve esthetics, they do not provide the same level of protection as a crown for root canal–treated teeth. The patient was fully informed about the limitations and still opted to proceed.

At the end of the day, dentistry is about giving options, educating patients, and respecting their decisions while achieving the best possible result.
