"Doc, Bakit nawawalan ng boses o napapaos ang ibang pasyente pagkatapos operahan ang Thyroid?"

Ang Vocal Cords natin ay nacocontrol natin gamit ang ating "Recurrent Laryngeal Nerve". Ito ay isang ugat galing sa ating utak na nagpapadala ng signals sa ating Vocal Cords kapag gusto natin magsalita.

Ang Recurrent Laryngeal Nerve ay nasa likod ng ating Thyroid Gland, at kung may Goiter tayo, minsan naiipit na ito, o minsan kinakain na ng mga bukol. Pag operahan ang Goiter, pwedeng masaktan o mamaga ito, kaya minsan may mga pasyente na napapaos pagkatapos ang Thyroidectomy.

Kailangan maging maingat ang isang surgeon upang iwasan ang pag-injure ng Recurrent Laryngeal Nerve. Magpaopera sa isang expert na sanay na magopera ng Thyroid Gland.

Where does Breast Cancer most commonly spread?

When Cancer spreads to other parts of the body, this is called metastasis.

When Breast Cancer spreads, the most common location of metastasis are the vertebrae- or the back bones. This can present as severe back pain. If left untreated, this can severely weaken the bones, leading to fractures. Almost all other cancers can also spread to the bones- like thyroid, lung, liver, prostate, and colorectal cancer.

Breast Cancer can also spread to the lungs and liver- and this can present as difficulty of breathing or liver failure.

Breast Cancer can metastasize when left untreated- early detection can help prevent urgent management and treatment, sparing patients from the dangers of metastasis.

What is the definitive treatment for Goiter? Large Goiters that cause compression of the airway and esophagus, along with Malignant Goiters will need a thyroidectomy.

The thyroid is removed through a Kocher cervical neck incision. The Strap muscles are either divided or retracted, then the thyroid vessels are ligated. The Recurrent Laryngeal Nerve which controls the vocal cords is then identified and preserved, along with the Parathyroid glands which control the calcium levels in the body.

A simple Thyroidectomy usually takes 2-3 hours, and patients are usually discharged after 1 day. If the thyroid mass/ lesion has already invaded the lymph nodes and adjacent structures, a more extensive surgery will be needed.

This highlights the importance of early surgical consult, to spare the patient from a more morbid procedure.
