Can you stop an overdose death? Updated guidelines may help

The American Heart Association has updated guidelines to address the threat posed by opioid overdoses, particularly fentanyl, which kill more Americans than all other poisons combined. The guidelines address substances that cause cardiac arrest, when the heart suddenly stops beating. They include recommendations for the treatment of 12 common types of poisoning, including drug overdose, chemical exposure, and drug interactions. The latest life-sustaining technology, called extracorporeal membrane oxygenation (ECMO), is the most important new advance in the treatment of poisonings.

ECMO pumps blood when the heart is unable to do so, buying time for the poison to leave the body. It is primarily available in large medical centers but is saving lives. However, a doctor might encounter a life-threatening overdose of that particular drug once every few years, while he treats someone for opioid overdose almost every emergency room shift.

The guidelines reflect the fact that opioid overdoses are becoming common enough to upend traditional thinking about cardiac arrest patients. Co-author Dr. Maryann Amirshahi, an emergency medicine physician at MedStar Washington Hospital Center and a medical toxicologist for the National Capital Poison Center, said that many of these patients have healthy hearts.

In Denver, the primary problem is fentanyl, which people with addiction seek out instead of he**in, and fentanyl shows up in counterfeit pills sold as oxycodone or ecstasy. Victims can turn up anywhere, and the guidelines emphasize the importance of everyone being prepared.

The guidelines note the importance of fast action and educating friends, families, and close contacts of people at risk of opioid overdose, including those taking opioids legally to control pain. Naloxone treatment is now an over-the-counter medication that temporarily reverses the effects of opioids, restoring breathing and making the victim more awake and alert.

Despite having the same rate of bystander CPR as their white counterparts, Asian persons are less likely to survive cardiac arrest.

A new study published in the Journal of the American Heart Association has found that Asian adults in the U.S. are less likely to survive a cardiac arrest outside a hospital than their white peers, despite receiving CPR from a bystander. The study, which analyzed data from the Cardiac Arrest Registry to Enhance Survival (CARES) for nearly 279,000 out-of-hospital cardiac arrests involving Asian and white adults in the U.S. from 2013 to 2021, found that Asian adults were slightly older, more likely to be female, less likely to have a drug overdose as the cause of cardiac arrest, and less likely to have a heart rhythm that would benefit from an automated external defibrillator (AED). The researchers also found that Asian adults were about 8% less likely to survive to hospital discharge and 15% less likely to survive without neurological problems than their white peers.
