Maxillary Sinusitis

🔹 Inflammation/infection of maxillary sinus
🔹 Maxillary posterior roots are close to sinus → important in dentistry

Causes
🔹 Odontogenic: periapical/periodontal infection, OAC, displaced root, infected cyst/tumor
🔹 Non-odontogenic: URTI, allergy, nasal obstruction

Clinical
🔹 Cheek/maxillary pain, may refer to teeth
🔹 Worse on bending over
🔹 Maxillary posteriors may be tender but vital
🔹 Purulent nasal discharge, post-nasal drip, halitosis
🔹 Fever/malaise in acute cases
🔹 Chronic: dull fullness and persistent post-nasal drip

Diagnosis
🔹 Tenderness over maxillary sinus
🔹 Water’s view: opacity / air-fluid level
🔹 Panorex/CBCT helps find dental cause

Treatment
🔹 Treat source: RCT, extraction, close OAC
🔹 Amoxicillin / Augmentin if bacterial
🔹 Decongestants + NSAIDs
🔹 Chronic/refractory: FESS / Caldwell-Luc
