💬 Editorial: Simulation modeling indicates that using apolipoprotein B ( ) as a target for statin intensification yields the highest quality-adjusted life-years and remains cost-effective compared with non–high-density lipoprotein cholesterol (non–HDL-C) and low-density lipoprotein cholesterol (LDL-C) guided strategies.

Sensitivity analyses confirmed that not only identifies more patients eligible for intensification but also more clinically meaningful residual cardiovascular risk. These findings support the revised 2026 ACC/AHA dyslipidemia guideline approach and expand the evidence for -guided lipid management.

Is this a new promising treatment for osteoarthritis?

For years, the orthopedic community assumed that GLP-1 drugs helped achy knees by helping people lose weight. Less load on the joint, less pain. It seemed like simple biomechanics. That story seemed intuitively obvious, and the STEP 9 trial published in NEJM supported it, 407 obese patients with knee OA experienced significant pain reduction and improved function with semaglutide 2.4 mg weekly versus placebo.

But… as I’ve been saying for a long time… this explanation was incomplete and didn’t match what I’ve seen in my office. If you’ve been reading this newsletter, you know I’ve written about the fact that OA patients on GLP-1 drugs report pain improvement long before any meaningful weight loss occurs. Weeks into treatment, before the scale has barely moved, people feel better. That timing never fit a purely mechanical story. Also, why do people with obesity get osteoarthritis of the hand more frequently than normal-weight people? It’s not the weight.
