As a precautionary measure, we will be screening patients and their companion/s by asking necessary questions pertaining to their travel history as well as current health status over the phone before we can confirm an appointment. Should we be able to confirm an appointment, the patient and his/her companion/s will be required to sign the form/questionnaire (which were asked over the phone) before starting the dental procedure.

We are reminding everyone that withholding vital information and failing to cooperate is PUNISHABLE BY LAW under RA 9271 “Quarantine act of 2004” and RA 11332 “Mandatory Reporting of Notifiable Diseases and Health Events of Public Health Concern Act".
