Dear Parents & Guardians,

Good day and Benedicite!

Please be informed that the Legazpi City Health Office Vaccination Team will be in St. Agnes Academy Gymnasium on August 20, 2022 (Saturday) 9:00 AM – 3:00 PM for the 5th Phase of COVID-19 Vaccination at St. Agnes Academy.
We encourage all students and family members who are unvaccinated and those who have not received booster shots to avail of the opportunity for your convenience.
The PINAS LAKAS vaccination strategy of the Department of Health focuses on strengthening the immune system of the populace by providing vaccine shots for specific age group.
As per DOH Guidelines, the recommended COVID-19 Vaccine shot for specific age group are as follows:
Pediatric 5-11 years old: 2 doses (1st and 2nd priming doses)
Pediatric 12-17 years old: 2 doses (1st and 2nd priming doses)
and 1 booster shot.
Adult and Senior Citizen: 2 doses (1st and 2nd priming doses)
and 2 booster shots (1st and 2nd booster).

Documentary Requirements:
• Pediatric Age Group 5-11 yrs old and 12-17 yrs old:
First Dose, Second Dose and Booster:
(1) Must be accompanied by Parent (bring any valid ID) or
If accompanied by guardian (bring Barangay Certification Proof
of Guardianship/Special Power of Attorney for Guardian),
(2) Birth Certificate/ Baptismal Certificate (with photocopy),
(3) If with Comorbidity – Medical Certificate using DOH Template.
(4) Pediatric Booster (12-17 years old): Vaccination Card

• Adult and Senior Citizen (18 yrs old and above):
First Dose: Any Valid ID
Second Dose and Booster: Vaccination Card and Any Valid ID

St. Agnes Academy, Inc. Legazpi City supports DOH's BIDA BAKUNATION. We are an Institution observing proper health protocols and our faculty and personnel are now 70% vaccinated with COVID - 19 vaccines.

Free Online Medical Consultation

I. Who can avail?
All students, faculty and personnel of St. Agnes Academy for AY 2021-2022

II. Consultation Schedule:
• Monday, Wednesday and Friday - 1:30 to 3:30 P.M.
• No consultations during holidays.

III. How to avail?
• Parents or guardians can set up an Online Appointment
• Appointments will be scheduled on a first notification first served basis.
• Only a maximum of 5 patients per day will be accommodated.

IV. Steps in setting up an online appointment with the school physician:

2. Click this Google Form link for Online Consultation and fill out the needed data.

3. You will receive a confirmation message with the date and time for your Online Medical Check Up.
4. On the day of the check-up you will receive a zoom meeting link in your FB messenger account.
5. Please log in to the meeting 5 minutes before your scheduled time.
6. Prescriptions and/or laboratory requests will be sent to your FB messenger account or E-mail address.
